A release of 36 bbl brine water was reported to the SWRO at 1347 by Operator representative Dan Chimino. Release discovered on 9/3 at 1220. No waterways reported to be affected.Upon receipt of spill report, Dept contacted Opr rep Dan Chimino at 1500 via phone. Mr. Chimino stated that early this morning, lightning struck the brine tank, igniting it and causing it to burn down to the base. The last well check was yesterday (9/2) in which there was 36 bbl brine in the tank at that time.Opr is currently removing any remaining fluids at this time and is in the process of contacting a contractor to pursue remediation efforts. Opr believes fluids may have traveled slightly downhill. No nearby waterways.Opr appears to be in compliance with current policy at this time. A FUI will be conducted in the future to assess any environmental impacts and/or issue any associated viols as necessary.
2302099 Primary Facility 09/05/2014 Incident- Response to Accident or Event Violation(s) Noted On-site inspt in response to admin inspt 2301651 for reportable release of 36 bbl brine from plastic tank.Release reported to be result of lightning strike igniting tank, causing tank to burn down to base.At time of inspt, area of previous tank location covered with plastic.Ground and remaining production equipment charred.Immediate perimeters of charred areas surrounded by discolored/stressed vegetation.Stressed vegetation does not appear to extend down nearby hillside, appears localized to well site.
2314757 Primary Facility 10/21/2014 Follow-up Inspection Outstanding Violations - No Viols Req'd FUI in response to viols issued under record 2302099 (enf ID 314995) for the 9/3/14 reportable release of 36 bbl brine water to ground.At time of inspection, contaminated soils excavated into single large pile, wrapped in liner awaiting proper disposal.All visibly impacted materials appear removed.Area of excavation surrounded by orange security fence.Site not yet temporarily stabilized.Per current regs, upon temporary cessation of an earth disturbance activity or any stage or phase of an activity where a cessation of earth disturbance activities will exceed 4 days, site shall be immediately seeded, mulched, or otherwise protected from accel E&S.Dept recommends Opr temp stabilize site in order to prevent any additional pollution events from occurring.
