1952107 Primary Facility 02/18/2011 Routine/Complete Inspection No Violations Noted On site with DEP WQS, Matt Shope to conduct an inspection of the pit. I observed that the pit has been completely filled in. Company Man, Dan Williams, informed me that the pit had been backfilled and encapsulated between 2-11 and 2-13-11. The drill cuttings were solidified the week of 2-7-11. There were no violations observed during the inspection. 2312489 Primary Facility 10/10/2014 Incident- Response to Accident or Event Pending Residual producation fluid spilled to surface of pad at fluid transfer hose connection. Transfer hose connection point was constructed outside containment structure for production fluid tanks with minimal secondary containment. Production Tanks collect fluid for all three producing wells at centralized location on upper Loomis Pad. Excavation and confirmatory sampling analysis has been planned to be conducted through Leidos Environmental.
2314754 Primary Facility 10/10/2014 Incident- Response to Accident or Event No Violations Noted Upon completion of the inspection at the lower Loomis well pad, we proceeded to upper Loomis well pad to investigate an area of staining of the pad surface, mentioned by Mr. Gregory during the Loomis lower pad spill investigation. I observed an area of staining on the pad surface in a relatively close proximity to the production fluid tanks containment system. The vacuum truck hose connection port utilized to offload production fluid from the tanks is housed outside of the production fluid tanks containment on a slab of concrete on the surface of the pad. The connection port is minimally contained. It is believed that as hoses from vacuum trucks are disconnected upon offloading production from the tanks, fluid leaked to the surface of the pad. Mr. Gregory mixed distilled water with pad surface soil to obtain a conductivity reading in where the staining was observed.
2327563 Primary Facility 12/09/2014 Routine/Complete Inspection No Violations Noted Remarks: The Department Arrived on site at approximately 13:30 on December 09, 2014 at the Loomis Well Pad (ESX10-115-0010) to conduct an inspection of the ongoing contaminated material removal. The SWN employees on site were Tracey Gregory and Tom Chitwood. Renee Andrews of Leidos was on site as well.

2445785 Primary Facility 2016-01-29 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:10 p.m., the cellar was empty, the (20x13) vent pipe was closed, the (13x9) vent pipe was closed, the (9x5) vent pipe was open and set up with a Taylor plug, the production casing was set up with a Taylor plug, there was about 360psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:40 p.m.

2822999 Primary Facility 2019-01-07 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Loomis well pad on January 7, 2019. The weather was overcast and 20 degrees F. There was no precipitation occurring.

The access road was long, passing two separate well pads. The first well pad was small and compact. The landowner was cutting firewood at the edge of the pad, with no adverse erosional effects. The access road and pad surface were both solid and stable.

The cut and fill slopes of this first well pad had good vegetative cover and there were no erosional problems noted.

The access road wound its way through a second gate to a scond well pad. The well pad contained the Loomis wells as well as an impoundment. The access road and pad surface were both well constructed and stable. The impoundment had at least 3 feet of freeboard. The fence surrounding the impoundment was intact and had a life preserver and rescue ring on the surrounding fence.

The slopes surrounding the upper Loomis pad were well vegetated and stable. There did not appear to be an
