Erick Flowback Services LLC was the subcontractor conducting flowback operations at the site. Items surveyed during the inspection included the secondary containment system for the flowback equipment, the gas buster tanks and frac tank battery next to the Pad berm used to store flowback water, the production containment system and the ground surface of the Pad. I observed several small petroleum sheens, rainbow in color, on the containment near the well heads. The containment systems for the gas buster tanks and frac tank battery consists of (40) mil HDPE liner, an (8) ounce layer of felt below the liner, and an (18) inch corrugated pipe berm. There were areas of compromised integrity observed in the liner in the frac tank containment. Overall, the containment systems at the site appeared to be in good condition.

I took a walk around the site and everything looked OK.
2474898 Primary Facility 2016-03-10 Routine/Complete Inspection No Violations Noted The Department was on site to inspect the Trecoske North Well Site(ESX11-115-0090). The wells associated with the pad are as follows:

There was evidence of earth work on site, on cut slope side, from last growing season, however work was mulched and stable.

The remainder of the site was stable and well vegetated with no apparent erosional issues. The last company inspection of the erosion and sedimentation controls of the site was on 3/7/2016.

There were no violations.

2511250 Primary Facility 2016-08-15 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:40p.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) valve was closed, the (9x5) vent pipe was open and venting into a tank. There was 2056psi on the production casing and 2116psi on the production line. The production casing was connected to the production line with high pressured connection. There was a chain and lock on the main production valves. I took a walk around the location and everything looked OK. It appeared the wells were recently serviced because there was rig mats stacked and on containment, and a role off container with used containment in it.

I left the site at 13:25p.m.

2544289 Primary Facility 2016-11-03 Site Restoration No Violations Noted The Department was on site to inspect the Trecoske North Well Site(ESX11-115-0090). The wells associated with the pad are as follows:

During last growing season, there was work performed on the cut slope side of the pad. This has all grown in well, and the site had in excess of 70% vegetative cover. All BMPs have been removed, and there were no erosional issues with the site at the time of inspection.

I recommend APPROVAL of this NOT.

2602062 Primary Facility 2017-06-07 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:25a.m.

The cellar was full of water with no bubbling noticed. The (9x5) annulus vent pipe was open and venting into the production tank. There was 1232psi on the production casing and 1219psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:00a.m.

2688937 Primary Facility 2018-01-30 Routine/Complete Inspection No Violations Noted On January 30, 2017 I inspected the Trekoske well pads. The access road was very solid and stable. The cut and fill slopes were well vegetated where not stone lined. The wells were all clearly and properly labeled and the pad surface was solid. The surrounding farm field edges had been previously undergoing enhancements and cleanup by the landowner near the access road, but the work has been completed and is now growing well and stable. There were no erosional issues noted. The wells associated with this site are the

There were no violations observed

2709566 Primary Facility 2018-03-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 4.5' below grade in the cellar and there was no bubbling noticed. There was a 500bbl flow back tank connected to 5 out of the 6 well on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2810604 Primary Facility 2018-12-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 2' below grade in the cellar and there was no bubbling noticed. There was a crew on site working on upgrading the electrical system. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
