2593946 Primary Facility 2017-05-16 Routine/Complete Inspection No Violations Noted On Tuesday, May 16, 2017, I conducted a routine-complete inspection of the Hammel UT 2 Well Site located in West Bethlehem Township, Washington County.

The Act 9 sign was in place and clearly legible at the site entrance which is secured with a locked gate. A emergency response plan and material safety data sheets were available on-site. The access road and well pad are in good condition and the site is stabilized with vegetative growth. Some rill and gully formation were observed on the access road. The operator should continue their periodic monitoring program and address any deficiencies, signs of accelerated erosion and/or sedimentation upon identification.

The The gas well appears to be in good working order and the earthen berm secondary containment structure which is partially vegetated appears to be performing as intended. No signs of any previous release were apparent.

No violations were noted during the inspection.
