As reported by Shell, approximately 100 barrels of produced water leaked from a tank into the secondary containment. The spilled fluid was removed, and Shell plans to have the rest of the tank emptied and then repaired. I arrived at 13:00; no personell onsite. The area around the produced water tanks was inspected. No evidence of leakage to the pad surface was observed.

Approximately 4 inches of snow cover on site. Electrical conductivity of puddles on the pad surface ranged from 325 to 570 uS/cm. Due to snow cover, the vegetative cover on iste was not visible. No evidence of accelerated erosion or sedimentation was observed.

2853534 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. Seven wells are staged for production. The wells have well tags which say Shell Appalachia instead of Rockdale. The Department recommends replacing the well tags. Some bubbling was observed coming from a green pipe attached to the well head of the 5H well.

Seven GPUs are present and appear to be in good condition overall. Two steel double-walled produced water tanks are present, labeled, and are in good condition. One white plastic produced water tank is present within steel containment with plastic lining. A methanol tote was also present within the containment.

Two infiltration basins are present to the west of the pad. Significant accelerated erosion is present downstream of the outlet pipe on the northwest infiltration basin. No other E&S issues noted.

No violations noted.

2856248 Primary Facility 2019-03-14 Routine/Complete Inspection No Violations Noted Weather was 28 degrees and cloudy. There were no Rockdale Marcellus representatives on location at time of inspection. Well was in production. Well had digital pressure monitoring equipment that had no read out. Well had 3 annulus vents plumbed to surface, 2', 1' and 3/4'. 100% LEL and 100% methane was detected on the 2' vent at time of inspection. 0% combustable gas was detected on the other 2 vents or in any area around the well at time of inspection. No violations noted at time of inspection.
