BOP test - 12/23/11, some drop-off - ? Comp. Man wasn't worried about it. 9 5/8" @ 2000' w/ 870 sx Stand, Cement w/ Gas Block, 15.6 ppg, 60 bbls back to surf, no packers. Discussed need to patch a few holes in plastic containment, keep 2' freeboard in waste tanks, and check on flare unit, light is not flashing- is it working? Checked cellar. Saw bubbles, 1 bubble every 1 to 2 seconds. Need to investigate further.

On site to check if bubbles in cellar are still present and if they are methane. No bubbles are present. Flare light is flashing, it was unplugged. Holes in containment have been patched. Comp. Man wil continue to monitor the cellar for bubbles and will let us know if he sees any further bubbling.

2039326 2012-01-31 Pending While inspecting the N BRA 2H, saw bubbling in cellar - full of water. Checked w/ Altair 5x gas meter - high CO(1000+ppm), about 40 ppm H2S, 100 comb gas. Had them pump out cellar. Hvy bubbling was from open valve on 5 1/2" x 9 5/8" annulus. However, I then noticed areas of less bubbling, in particular, at the spot where bubbles were previously seen while drilling, as noted in inspection report. Not sure where the bubbles were coming from, possibly from inside the 20" conductor. Will need to revisit the site after rig has moved.

2136524 2013-01-31 No Violations Noted Cellar is full of dirty water. Vents on 5 1/2" prod(not open) csg and 5 1/2 X 9 5/8" annulus(open) have pressure guages w/ 0 psi.

The 9 5/8 X 13 3/8" annulus is vented - comb = 20% LEL, Meth = 0 The 20 x 13 3/8" is capped and vented - Comb = 0, Meth = 0 Guage on top of wellhead (5 1/") has 340 psi.
