Proof of the neighboring property owner¿s written consent was provided 01/26/2011 and a new plat was submitted. Drilling was taking place at the time of inspection. A site ID and E&S plan were posted on site. E&S controls were in place according to the plan. The access road was in good condition. The drilling pit appeared to be in good condition. No spills, leaks, or accelerated erosion were observed at the time of inspection.

No spills, leaks, or accelerated erosion were observed at the time of inspection.

1961888 2011-03-25 No Violations Noted On March 25th, 2011 I conducted a routine inspection of the above site. Drilling and frac were completed. Frac tanks were still on site. A site ID and E&S plan were posted on site. E&S controls were in place according to the plan. The access road was in good condition. The drilling pit was closed. The site was beginning to be put back. No spills, leaks, or accelerated erosion were observed at the time of inspection. The operator should continue to inspect the E&S controls routinely and after each runoff event until the site is completely restored and 70% vegetative cover is achieved.

2105626 2012-10-04 No Violations Noted John Ley WQS SWRO, was on site to complete a routine inspection. Drilling was completed and the well appeared to be equipped for production. Brine tank located onsite appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in on the access road or well-site. The site had at least 70% vegetation on the hillsides. The only minor issue was that the impoundment area around the tank was holding water and should be drained to avoid overcapacity in case of a spill.
