2653586 Primary Facility 2017-10-03 Routine/Partial Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The purpose of the inspection was to discuss redline changes to the site. Rice requested Department approval to shift BMPs toward the LOD to allow PCSM to be built during the initial construction phase to limit future earth disturbance. The redline changes were approved.
2667003 Primary Facility 2017-11-28 Compliance Evaluation No Violations Noted The Act 9 sign was in place and all required documents were present at the site. A large portion of the site was temporarily stabilized. An NOV was previously issued in response to a BMP failure that caused sediment to leave the LOD and enter waters of the Commonwealth. Repairs had been made to the BMPs and there was no sediment leaving the site. There were no violations observed at the time of inspection.

2712334 Primary Facility 2018-03-23 Routine/Partial Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The land owner requested EQT add a culvert in an access road diversion channel and gravel in the adjacent area to allow better access to their property with farm equipment. Due to the addition of impervious area, it was recommended to EQT that they contact Department engineering staff. Redline approval was not granted.

During the inspection EQT informed the Department a slip had occurred upslope of the rain garden on the north side of the site. The slip material was contained in the rain garden. No slip material left the LOD.

There were no violations observed at the time of inspection.
