0845773986 2016-09-20 Temporary Abandoned Annular Disposal No Violations On site inspection accompanying Inspector Austin Gutridge found the well was equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing, 1.5 inch diameter tubing, rods, and a pumping unit. The well is capable of production but the valves are closed. No Annular disposal was hooked up or in use.
1197481118 2016-09-20 Production Wells Violations Noted The purpose of my inspection today is a status check on a temporarily abandoned annular disposal well. During my inspection the well was equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing, 1.5 inch diameter tubing, rods, and a pumping unit. The well is capable of production but the valves are closed at this time. This well is not disposing brine through the annular disposal method. There is not an annular disposal connection plumbed up to the annulus. The production equipment for this well, located within an earthen dike, consists of one 100 barrel tank, and one separator. No identification was present. Small amount of oil was found surrounding the base of the 100 barrel tank due to small holes in the base of the tank. I attempted to contact B & E Resources on 9/20/16 at 4:50 PM via pone call at the phone number 740-410-1090 but was unsuccessful. I left a message with my contact information. The oil shall be removed from the dike and the contaminated soil shall be removed from the dike and hauled to an approved landfill. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number, county, and an emergency telephone number.
1476711655 2016-10-17 Production Wells No Violations The purpose of my inspection is a follow up on a compliance notice numbered -1197481118 that was issued 9/21/2016. Inspection on 9/20/2016 found a small amount oil surrounding the 100 barrel tank due to holes in the base of the tank. The oil was contained within the earthen dike. My inspection today found no contamination within the dike. A new 100 barrel steel tank was in place and the dike walls have been reconstructed. During my inspection the pumping unit was in operation and an up to date chart was in the chart meter with the date reading 10/12/16. This well is in production with no violations.
