2170180 Primary Facility 2013-05-28 Routine/Complete Inspection No Violations Noted
2463866 Primary Facility 2016-03-08 Routine/Complete Inspection No Violations Noted no methane detected at annuli. Some minor staining on or around the well.
2465536 Primary Facility 2016-03-16 Routine/Complete Inspection No Violations Noted At the time of the inspection, 6 wells onsite were in production. The access road, well pad, and reserve pit remain installed as constructed. The Fresh Water Impoundment (FWI) has been removed and that area has been returned to approximate original contours. The entire permit area is currently well vegetated. No Violations Noted.
2514920 Primary Facility 2016-08-24 Routine/Complete Inspection No Violations Noted Routine Inspection; 8/24/2016.

I entered the site off SR 87 at 11:37 to evaluate Inflection Energy’s plan to discharge from the FWI (Fresh Water Impoundment) onsite. At the time of the inspection 6 wells onsite were in production. The dewatering plan indicated Option A will discharge water via a sprinkler system at approximately 57 GPM while Option B involves a filter bag dewatering structure to apply approximately 63 GPM. Overall, both area are well vegetated. The infiltration testing included with the plan demonstrates known rates of infiltration which do help evaluate application rates. The proposed discharge locations and application rates appear to be reasonable but I recommend monitoring the dewatering rate and adjusting accordingly. Please remember to notify the Department prior to discharging as indicated in your plan. No Violations Noted.

2780120 Primary Facility 2018-09-26 Routine/Complete Inspection No Violations Noted Six wells staged for production phase. Five each water separators within containment. No leaks detected from production equipment. No violations noted.
2840513 Primary Facility 2019-02-14 Routine/Complete Inspection No Violations Noted I arrived on site with Dave Hooker of the Department's Air Quality Program. Site was covered with icy snow. Production tank containment had 2-4" of ice and snow in it; one of the tanks had some minimal rust streaking, but the other two tanks appeared to be in pristine condition. Wells were tagged.
