2617046 Primary Facility 2017-07-18 Routine/Partial Inspection No Violations Noted On 7/18/17 at 1323 hrs I attended a preconstruction meeting for the Sweden Valley 107 well site. JKLM representatives, Jeff Long and John Manuel, and J.L. Watts representative, John Watts, were present. CB&I developed the Erosion and Sediment (E&S) control plan and J.L. Watts is the construction contractor.

We reviewed the E&S drawing. Site drainage is to an unnamed tributary to Nelson Run, which is designated as a High Quality stream. A majority of the access road was constructed under the Sweden Valley 106 ESCGP. The access road for the 107 pad will come off the existing access road and is 200 feet in length. The site will consist of a well pad (350 feet x 500 feet) and a sediment basin located off the northwest corner of the pad. The pad will be soil cemented for stability. The E&S drawing shows that a wash rack is to be installed on the access road. JKLM would prefer to install an additional 50 feet of rock constructed entrance in lieu of the wash rack; this is an acceptable alternative when a sediment

2633715 Primary Facility 2017-09-06 Preoperation Inspection No Violations Noted On 9/6/17 at 0938 hrs I conducted a preoperational inspection at the Sweden Valley 107 well site. It was mostly cloudy and 61 degrees F.

I observed the site and the routine Erosion and Sediment (E&S) control inspection log. Earth disturbance is complete. The pad surface and access road will be soil cemented and rocked within the next week or so. E&S controls are installed and disturbed areas have been stabilized with seed and mulch or erosion control blanket; seed has germinated in some areas.

Cellars have not yet been installed. Well permits for the Sweden Valley 107 1HU, 2HU, 3HM and 4HB have been issued.

The last routine E&S inspection was conducted on 8/30/17. No violations are cited during this inspection. I concluded the inspection at 1052 hrs. TLT

2650322 Primary Facility 2017-10-19 Routine/Complete Inspection No Violations Noted Well was drilled and cemented on 10-13-17. Well is conductor only. This well as well as 105-21896 are utica formation wells. The producing formation choice in this inspection format did not allow me to make that choice when submitting this or the inspection for 105-21896.
2695944 Primary Facility 2018-02-15 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the JKLM Sweden Valley 107 well pad located in Sweden Twp., Potter Co. on 2/15/2018 @1300 hours. The weather was 48 degrees and sunny causing accumulated snow to melt and the pad surface and access road to be very wet/muddy. JKLM representatives Jon Manuel and Ron Hall were present for my inspection. The H&P 500 flex rig is onsite and is currently drilling the 1HU well. The rig was at a depth of 11,474’ feet at the time of my inspection. Mr. Hall stated no issues have been encountered. The rig will be drilling 4 wells while onsite the 1HU, 2HU, 3HM and 4HB. Fluid was observed accumulating along the eastern side of the pad-wide containment. Fluid in containment had a field measured conductivity of between 6.82 mS/cm and 8.28 mS/cm. A puddle on the ground surface approximately 1-2 feet off the edge of containment had an elevated conductivity of 7.23 mS/cm. Additional screening of the surrounding area showed the elevated area to be isolated. A vac truck
2719485 Primary Facility 2018-04-17 Routine/Complete Inspection No Violations Noted At the time of this inspection the drilling of this well was complete and the drilling rig was rigging down and being moved.
2730072 Primary Facility 2018-05-15 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the JKLM-Sweden Valley 107 well pad located in Sweden Township, Potter County. The weather was 77° mostly cloudy with severe thunderstorms in the area, winds W 12 mph, 61% humidity. Chris Wiley, JKLM was present onsite.

JKLM was preparing to frack the 1HU, 2HU, 3HU and 4 HB with contractor Keane Frack. Mr. Wiley stated they were hoping to begin fracking later this afternoon. The onsite AST, containment and E&S controls were observed. No puddles or staining was observed around the AST. All duckponds observed appeared dry. All observed puddles, on the pads surface, were measured for field conductivity and found to be within normal range. Rain water was accumulating inside the northern edge of the primary containment. Mr. Wiley was informed of the finding. Due to severe thunderstorms and the potential for heavy precipitation, storage capacity of primary containment should be monitored.

2738165 Primary Facility 2018-06-06 Routine/Complete Inspection No Violations Noted The 1HU had completed all 26 stages at the time of this inspection.
2740709 Primary Facility 2018-06-06 Routine/Complete Inspection No Violations Noted The DEP conducted an INCDENT inspection of the JKLM – Sweden Valley 107 well pad located in Sweden Twp., Potter Co. @ 1045 hours on 6/6/2018 in response to spill#334721 reported to the DEP on 6/5/2018 @1534 hours. The weather was 58 degrees and overcast.

A representative from JKLM contractor Whipper Hill Consulting was onsite conducting a grid screening of the impacted area. The following information was provided.

On 6/5/2018, sometime between 1000 – 1330 hours, a contractor punctured the pad-wide containment beneath the sand boxes located along the northern edge of containment. An unknown volume of fluid exhibiting an elevated conductivity flowed to the northern edge of containment, thought to be the “low spot”, where it exited the puncture causing a puddle to form outside of containment. The representative stated the puddle had a field measured conductivity of >20mS/cm. A vacuum truck was dispatched to remove the standing liquid from the pad surface.

2745354 Primary Facility 2018-06-25 Routine/Complete Inspection No Violations Noted At the time of this inspection the hydraulic fracturing of this well was complete.
2782160 Primary Facility 2018-10-01 Routine/Complete Inspection Pending The DEP conducted a routine inspection of the JKLM - Sweden Valley 107 well pad located in Sweden Twp., Potter Co. on 10/1/2018 @1240 hours. DEP Env Trainee, was present onsite for the inspection. The weather was 73 degrees, partly sunny.

Company personnel onsite appeared to be repairing a dehydration unit that had sprayed glycol onto the pad surface on 9/28/2018. The glycol release was below the reportable limit (but had been reported to the DEP), and appeared remediated by the time of this inspection.

The pad surface appeared very wet and muddy around the well heads, AST and main traffic areas. Puddles on the pad surface, near the production tanks, tested for field conductivity with an Oakton Multi-Parameter PCTestr and a Groline soil conductivity meter measured ~1000 uS/cm.

A number of flowback tanks were arranged, on secondary containment, along the eastern side of the pad. Standing fluid in the secondary containment had a field measured conductivity of "Over Range" or >20.00mS/cm. A large wet/damp

2795447 Primary Facility 2018-10-29 Administrative/File Review Violation(s) Noted An Administrative inspection was conducted 10/29/2018 in response to receipt of results from sample 4427-170 dated 10/1/2018, referenced in IR#2783174.

The sample was collected from high conductivity fluid located on the pad surface, outside of containment. The sample results show elevated levels of barium, strontium and chlorides. These results are consistent with the release of a pollutional substance to the pad surface.

2802122 Primary Facility 2018-11-13 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was drilled,stimulated, flow-backed and was in production status. Flow-back is done before well is put into production as part of production preparation. The pressure reading on a mechanical gauge on this well at this inspection was 1700 pounds per square inch. Flow- back was started on 9/12/18 and completed on 10/23/18. No violations were noted at this inspection.
2820215 Primary Facility 2018-12-20 Routine/Complete Inspection Outstanding Violations - Viols Req'd The DEP conducted a routine inspection of the JKLM Sweden Valley 107 well pad located in Sweden Twp, Potter Co on 12/20/2018 @1230 hours. The weather was overcast, 34 degrees.

The well pad is currently setup for flowback/production. Personnel were present onsite to monitor transfers from/to the Aboveground Storage Tank (AST). A temporary fluid line was observed connecting the production tanks to the AST, but did not appear to be in use. Fluid with a field measured conductivity of "Over Range" or >19.99 mS/cm was observed in the production tank containment. Puddles screened on the pad surface measured <1000uS/cm.

Violations from an inspection dated 10/29/2018, remain outstanding at this site from a previous release. Site remediation has been performed and a Remediation Action Completion Report has been submitted to DEP’s Environmental Cleanups and Brownfields program for review. The outstanding violations will be closed once attainment with Act 2 standards has be confirmed.

2846822 Primary Facility 2019-02-28 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the JKLM - Sweden Valley 107 Well Pad located in Sweden Twp, Potter Co. on 2/282019 @1230 hours. The weather was 23 degrees, sunny, winds N 4mph,63% humidity, 2-3" of snow deposited over the previous 24 hrs. The well pad appears to be setup for flowback/production. 4 wells were observed connected to sand traps and a gas buster. 17 orange roll-off tanks were observed on containment on the along the south side of the well pad.
2848027 Primary Facility 2019-03-05 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. There are three (3) annulus vents that are vented to surface. These annulus vents were checked for combustible gas on each annulus vent. Results of this testing were 0 LEL ( lower explosion limit) and 0 Methane on vent 18 x 13 and 0 LEL and 0 Methane on vent 13 x 9 and 0 LEL and 0 Methane on the 9 x 5. A gauge on the well showed a pressure of 1500 psi ( pounds per square inch ) . No violations were noted at this inspection.
