2491306 Primary Facility 2016-06-08 Incident- Response to Accident or Event No Violations Noted Inspection conducted in response to spill of 20 gallons of diesel fuel reported by Talisman to DCNR (lessee). This has been assigned Spill Notification No. 320223 in the Department's Complaint Tracking System.

The spill occurred during fuel transfer operations near the drilling rig on site. The spill was to containment and not the ground surface; no spilled fuel left containment. The spill was promptly cleaned up, which included the removal of two deck mats and the vacuuming of fluids from the containment beneath that location. The containment was inspected and no holes or tears were identified.

During my inspection no additional areas of staining, diesel odors, or spilled fuel were identified elsewhere on the well site.

This inspection has been filed under Permit No. 117-21799.

2630893 Primary Facility 2017-08-30 Routine/Complete Inspection No Violations Noted I met Shaun Smith of Repsol on location. Site is being prepped for frac; 480K Dyna-Tank was in place and had been filled. Secondary containment for the tank was complete, and most of the site-wide containment was in place and bermed. Shaun stated that they should be pressure testing by Saturday.

2632407 Primary Facility 2017-09-05 Routine/Partial Inspection No Violations Noted I met with Company Man Sheridan Smith who expects Halliburton to be finished fracturing the 4 Marcellus wells in about 25 days.

The highest electrical conductivity reading I got from the pad surface with an Oakton PCSTestr 35 was 0.48 mS/cm. Fresh water is contained in an onsite impoundment.

2643412 Primary Facility 2017-10-03 Incident- Response to Accident or Event No Violations Noted I met with Glen Cerny (Ensign) on site, and he showed me the area of the pad where the diesel spill occurred 5 days ago (photo 4). We also discussed the bridge (photo 1). He said they will remove the liquid, then replace the bridge.

They plan to start drilling out plugs and flowing back the wells this week.

2649638 Primary Facility 2017-10-18 Routine/Complete Inspection No Violations Noted I met Tony Baker when I arrived on-site. Basic Well Services workover rig was on location, pulling tubing from the 4H well. Containments were in good condition; however, I noted on the east side of the pad, the liner was no longer running up onto the soil berm, but stopped short. Tony told me he would have it repaired. AST containment also appeared to be well maintained and sound.
