2517298 Primary Facility 2016-08-01 Complaint Inspection Pending On 8/1/16, I performed a complaint inspection at the Wiggin Out site in response to complaint 321416 and made the following observations:

•Rice Drilling representative Jonathan Davis met me on site to inspect reports of large amounts of sediment and water leaving the site. •PCSM control construction was ongoing at the time of inspection. Wet ponds were completed on both sides of the site. No violation. •Disturbed earth around ongoing construction was not stabilized at the time of inspection. No violation. •Runoff water and sediment were found to have left the site LOD in two locations below the wet pond on the east side of the pad. Materials outside LOD extended into heavy brush as far as could be followed. Follow up inspection needed to verify extent of full impact. Perimeter CFS was overtopped or collapsed; lack of stabilization and improper tracking of soils in the adjacent areas combined with heavy rains are suspected to have led to the failure. Violations pending further investigation of impacts.

2519559 Primary Facility 2016-08-05 Complaint Inspection No Violations Noted On 8/5/16, I performed a second complaint inspection at the Wiggin Out site in response to complaint 321416 accompanied by WQS Supervisor John Carson and made the following observations:

•Inspection of the complainant’s spring and collection area found sediment and other indications that runoff from the site had directly impacted the complainant’s water supply. A temporary water supply was requested from the operator. •Runoff water and sediment from the site was found to have traveled around 100 yards outside the LOD from the breached perimeter controls found during the 8/1/16 inspection before entering the spring and stream system immediately downslope from the wet pond. This constitutes a violation of 35 P.S. § 691.401, 25 Pa. Code § 78.53, 25 Pa. Code § 102.4(b)(1), and 25 Pa. Code § 102.11(a)(1). •Inspection of the wet pond area found that all perimeter controls had been repaired and temporary stabilization had been implemented.

2535052 Primary Facility 2016-08-23 Complaint Inspection No Violations Noted On 8/23/16, I performed a complaint inspection at the Wiggin Out site in response to complaint 321903 accompanied by WQS Justin Najewicz and made the following observations:

•Inspections of the wet pond to the east of the site found insect and amphibious organisms and no indication of contamination. Conductivity, salinity, and pH were all within normal ranges for site conditions.

2746699 Primary Facility 2018-06-28 Routine/Complete Inspection No Violations Noted This well has been completed and is producing. Wells and site appear to be well maintained.
2759595 Primary Facility 2018-07-31 Routine/Partial Inspection No Violations Noted On 7/31/18 a routine partial inspection of the Wiggin Out well site was conducted in response to an operator request. The following was observed:

>>EQT Representative Jason Tyler requested a meeting to review plans to remove multiple sections of site CFS at landowner request to allow livestock access. Site vegetation suggests that CFS should be removed independent of landowner request.

>>Site appeared in generally good condition with no noted issues.

2777696 Primary Facility 2018-09-20 Incident- Response to Accident or Event No Violations Noted On 9/20/18 a complaint inspection of the Wiggin Out Well Pad was conducted in response to a report of a landslide at the site. The following was observed:

>>In the area of the reported slide disturbed soil does not appear to be a slide, rather it appears to be animal damage related. The disturbed area was found below the sediment pond north and east of the pad, clear water was found to be leaving the sediment pond via the drain. No violation.

>>Multiple small areas of animal sign and disturbed soil were found around the sediment pond.
