2582641 Primary Facility 2017-04-13 Preoperation Inspection No Violations Noted On Wednesday, April 12, 2017, I conducted a pre-operational inspection at the Bovinator Well Site located in Amwell Township, Washington County. The inspection was performed following an operator request in accordance with 58 Pa. C.S. Section 3258 (a.1). Mr. James Davis, representing Rice Energy (Rice), was also present during the inspection.

The Act 9 sign was in place and clearly legible at the site entrance and all required documentation was present on-site. The access road and well pad were in good condition. The stream crossing was in good condition and constructed according to the plan. At the time of the inspection, Rice was in the process of installing concrete safety barriers along the access road in the vicinity of the stream crossing for driver safety during inclement weather which will further protect the stream as well.

All perimeter erosion and sedimentation controls have been installed according to plan and are functioning as intended. The operator should continue their periodic monit

2601699 Primary Facility 2017-06-07 Drilling/Alteration No Violations Noted Butch’s Rathole Service auger rig

Drilled a 36” wellbore and set 26” conductor casing at 50 ft. on 6/5/17.

2756300 Primary Facility 2018-07-26 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #349 moved onto this well on 7/24/18 and resumed drilling at 87 ft. KB. The 18 5/8" casing was set at 331 ft. On 7/26/18 and cemented to surface with 25 bbls returned. Currently at 346 ft. and waiting on cement.
2759648 Primary Facility 2018-08-02 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #349. The operator drilled to 649 ft. A mine void was found from 543 ft. to 549 ft. The operator set 3 flash plugs to stop the flow of mine gas. The first plug was 80 bbls of cement. The second and third plugs were each 60 bbls. The 13 3/8" casing was set at 586 ft. on 7/30/18. The operator cemented the casing to surface with 80 bbls returned. The cement then fell 92 ft. The casing was then grouted to surface. Two baskets were set at 445 ft. A packer was set in the 13 3/8" casing from 446 ft. to 460 ft.
2765714 Primary Facility 2018-08-21 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #349 reentered this well on 8/10/18 and resumed drilling at 649 ft. The 9 5/8" casing was set at 2037 ft. on 8/13/18 and cemented to surface with 39 bbls returned. KOP was at 2080 ft. Just landed curve. Do not yet have depth numbers. Currently at 8265 ft. MD and drilling ahead.
2768696 Primary Facility 2018-08-29 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #349. EOB was at 8170 ft. MD (7469 ft. TVD). TD was at 14,891 ft. MD (7528 ft. TVD) on 8/25/18. The 5 1/2" casing was set at 14,876 ft. Estimated TOC at 5000 ft.
