0101816941 | 2015-04-16 | Plug / Plug Back | No Violations | Matheny Well Service ripped the 4.5 casing yesterday at 760 and pulled the casing OOH. Total depth was checked today and tools sat down at 612, indicating more cave-in in the well bore. 2 tubing was run to 584 and Formation Cementing pumped a 200 plug with 57 sks of class A cement with 2% calcium chloride. Tubing was pulled OOH to avoid getting hung in the hole and the plug was allowed to set for 1 hour. Tubing was then run to 340 and another 200 plug was set with 63 sks of class A cement with 2% calcium chloride. Tubing was then pulled to 136 and cement was circulated to surface with 57 sks of class A cement. These 3 plugs were set with the intent of creating 1 large plug from 584-0. Good circulation was achieved on all plugs set. |
0321768475 | 2016-05-24 | Final Restoration | No Violations | The purpose of my inspection today is a status check on a final restoration. During my inspection I witnessed no evidence of an oil and gas well and the site has been restored. Location has met 1509.072 B requirements. |
0327673745 | 2015-04-14 | Administrative Inspection | No Violations | Jon Schneider called and stated that Matheny Well Service was able to run tools to 1254, indicating most of the cement went downhole, close to where it was intended to go. Free pipe was found at 780 and will be ripped there tomorrow. The 4.5 casing will be pulled and 2 tubing will be run back in to set a rip isolation plug. |
0844693303 | 2005-05-16 | Production Wells | No Violations | Inspection this day found casing head well producing, current chart in meter, house gas meter also, tank has good IDs, no dike, old Tiger Oil well. |
0951391565 | 2015-04-07 | Plug / Plug Back | No Violations | On location to witness plugging operations for this well. I was accompanied by newly hired Inspector Matt Fulk. Matheney Well Service set 2 tubing at 2118. Formation Cementing pumped a 400 bottom plug using 30 sks of 2% gel cement, 2% calcium chloride, 30 lbs of LCM, and 6.9 bbls of freshwater displacement. Stonebridge Operating Co. elected to let the plug set up for 24 hours and will tag the plug tomorrow morning before ripping the 4.5 casing and plugging out. |
1429012356 | 2015-04-13 | Plug / Plug Back | No Violations | On location to witness plugging operations for this well. Matheny Well Service attempted to rip the 4.5 casing at 1581 for approximately 5 hours and was unsuccessful. Appalachian Well Surveys was brought in to shoot the casing off. The casing was shot at 1541, but the casing was found to be hung in the hole. Formation Cementing hooked up to the 4.5 casing and circulated a large amount of red rock debris from the backside of the casing, but the casing was still hung. It was then decided to set a 200 plug through the hole that was shot in the 4.5 casing. Formation Cementing pumped 45 sks of 2% gel cement into the casing and cement returns were seen at the surface before displacement started, indicating a hole in the casing. Formation flushed as much cement as possible from the hole, but it is likely that some still remains. The Maxton Sandstone began to flow oil and gas as cement was being flushed from the hole. The well was shut in and it went on vaccuum. The plug will be tagged tomorrow morning and a further course of action will be determined afterward. |
1981721811 | 2015-04-02 | Administrative Inspection | No Violations | Mr. John Schneider of Stonebridge Operating called to inform me of intent to plug and abandon this well after an unsuccessful attempt to service the well. Mr. Schneider will be applying for an expedited plugging permit in hopes to keep the service rig on location and plug the well next week. |
