0481739773 | 2012-01-04 | UrbanProduction Wells | No Violations | Well is shut in. Have received calls from landowner complaining that the well is idle. Operator has assessed well. The well is watered out, zero pressure on the tubing. |

1185003812 | 2014-10-06 | Final Restoration | No Violations | My inspection found the tank battery is still intact. Once the equipment is removed the tank battery needs graded, seeded, and strawed. Once vegetation is established and the flow line waiver is submitted I will pass final restoration. I contacted Kyle Ahbel from Enervest about finishing the restoration. |
1320238166 | 2011-10-18 | UrbanProduction Wells | No Violations | On site due to a call from the land owner that the well is idle. This is a rabbit well and it is shut in at the tubing and the 4.5 casing. The well has zero pressure and the last reported production was 550 MCF in 2010. I spoke to Joe Terpek from Enervest and notified him that the wellneeds produced or plugged. |
1382360768 | 2013-10-21 | UrbanProduction Wells | No Violations | My inspection found the well to be equipped with a plunger lift unit. The tank battery consists of a 210 BBL production tank, a 210 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition. A plugging permit has been issued for this well. |
1390395060 | 2014-01-16 | Plug / Plug Back | No Violations | I was on location to witness the plugging of this oil and gas well. Nicks Well Plugging is the contractor doing the plug job for Enervest. Nicks stripped the 2 3/8 tubing out of the well, rigged up, and attempted set the Clinton plug through the 4.5 casing with 25 sacks of cement and 50 pounds of cottonseed hulls. During displacement the 4.5 casing ruptured and started to circulate up the backside of the 4.5 casing out the 8 5/8 casing. They continued to circulate the well until the cement plug returned to surface. Nicks then tripped the tubing back into the well to set the Clinton plug. Nicks used 25 sacks of Class A cement with 50 lbs of cotton seed hulls to set the Clinton plug. The well was shut in until the following morning. |
1422289973 | 2015-01-26 | Final Restoration | No Violations | My inspection found the tank battery has been removed. Once a flow line waiver is submitted final restoration can be passed. |
