Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID
1904341 2010-08-10 No Violations Noted Proposed site staked. Site not constructed.

Request for waiver of distance requirement from wetlands associated with Little Mahoning Creek. Revisions to E&S plan requested and received. Spoke to operator representative Tom Baun and consultant Jesse Hickman about these revisions. Revised plan should protect wetlands from adverse impact during site/well development if properly implemented and maintained.

1925213 2010-10-27 No Violations Noted Site id sign in place. E&S plan on site. Well not equipped for production (not yet fraced). Spudded on 10/15/10.

Waiver of distance requirement from wetlands associated with Little Mahoning Creek approved on 8/10/10. Access road is stable. Access road entrance is adequately rocked. Areas of disturbance along roadway have been seeded/mulched. Culverts rip-rapped. No signs of accelerated erosion on access road. BMP's are in place in accordance with E&S plan on road and site.

  • Rain/water run-off has nearly washed out underneath the silt fence/hay bales in one small area on the down-slope side of the site.
Additional measures need to be taken to ensure E&S problems are avoided. Spoke to operator representative Tom Baun about this concern, who indicated the work would be done immediately and I would be contacted upon completion.

Drill pit is properly lined, appears sound, and has adequate freeboard. No violations found.
