2444799 Primary Facility 2016-01-27 Incident- Response to Accident or Event No Violations Noted On 1/27/16, I performed an incident inspection at the Ironman Pad in response to these concerns and identified violations including the following:

•My Inspection indicates that either portions of the initial slide or a new slide are currently outside the LOD and within a stream bed directly impacting an operator delineated stream. •A sump outlet shown in the plans extending south east from the southern corner of the pad had a pipe extension added conducting it an additional ~50 feet to a CFS dam not shown or redlined on site plans. Water was running under the CFS dam still carrying sediment and then continuing around perimeter CFS, out of the LOD and into the adjacent stream channel. •In the area where the LOD extends along a spur line NW of the pad a number of hay bales and sections of silt sock have been added in an operator delineated intermittent stream. •An erosion channel was found extending from the north side of the pad near the spur line into the intermittent stream bed. -->Further details in fu

2527017 Primary Facility 2016-03-08 Follow-up Inspection No Violations Noted On 3/8/16, I performed a follow up inspection at the Iron Man Pad regarding an NOV from 1/27/16 (enforcement ID: 333960/Inspection ID: 2444782) accompanied by DEP Engineer Emmanuel Nwaobilo and made the following observations:

•While the pipe extension from the sump outlet noted in the previous extension was removed and additional sections of sock and erosion matting has been added erosion damage outside the LOD and pollution to the Waters of the Commonwealth appears to be ongoing. This constitutes a continuation of the violations of 35 P.S. § 691.401, 78.53, 102.4(B)1, and 102.11(A)1 noted in the inspection from 1/27/16 by continuing in failing to design, implement, and maintain the site E&S BMP’s in such a manner that it prevents and minimizes erosion and sedimentation resulting in a discharge of sediment to Waters of the Commonwealth. •Erosion control measures and stabilization around the landslide that crossed the site LOD have been modified, however no observable effort has been made to remediate the

2527363 Primary Facility 2016-06-28 Follow-up Inspection No Violations Noted On 6/28/16 I performed a follow up inspection of the Iron Man site accompanied by DEP WQS John Parry and Rice representative Jonathan Davis in response to violations noted during inspections on 1/27/16 (ID: 2444782) and 3/8/16 (ID: 2527004) and made the following observations:

•The land slide located along the site LOD on the south eastern border of the site remained unremediated at the time of inspection and continues to impact the Waters of the Commonwealth. This constitutes a continuation of the violations of 35 P.S. § 691.401, 78.53, 102.4(B)1, and 102.11(A)1 noted in the inspection from 1/27/16 by continuing in failing to design, implement, and maintain the site E&S BMP’s in such a manner that it prevents and minimizes erosion and sedimentation resulting in a discharge of sediment to Waters of the Commonwealth. •The area within the site LOD directly up gradient of the slide may be showing additional ground movement but significant disturbance of the area by cattle made definitive assessment unfeasible

2527834 Primary Facility 2016-09-30 Routine/Partial Inspection No Violations Noted Routine/Partial inspection conducted to assess an ongoing issue at the site.
2657484 Primary Facility 2017-10-12 Follow-up Inspection No Violations Noted On 10/12/17 a FUI inspection was conducted of Ironman well site accompanied by DEP WQSS John Carson, DEP Engineer Emmanuel Nwaobilo, DEP Compliance Chief David McDermott, and various Rice Energy representatives in response to an operator request for a meeting regarding current and potential site modifications. The following was observed:

>>Inspection of the contested slide first noted during a site inspection on 1/27/2016 found that the slide had not been remediated but had not notably progressed. Potential remediation by stabilizing the slide edge and vegetating the area.

>>An additional slide was found in the vicinity of the previous slides below the south west corner of the pad. An erosion channel ran from the edge of the new slide out of the site LOD into the same area as the contested slide first documented in the 1/27/16 inspection.

>>Rice Energy representatives will propose additional measures to continue remediation of site issues.

2788169 Primary Facility 2018-10-11 Routine/Complete Inspection No Violations Noted On Thursday October 11, 2018 I conducted a routine-complete inspection at the Iron Man Well Site located in North Bethlehem Township, Washington County.

Gas well Ironman 7H (125-27720) and the associated infrastructure appeared to be in good condition and functioning properly. The gas well was tagged with identification.

No violations were identified during the inspection.
