2392192 Primary Facility 2015-07-13 Incident- Response to Accident or Event No Violations Noted Purpose of the inspection was to document a 63 gal calcium chloride release reported 7/7/15, which had been cleaned up. While on site, evidence of a new release was observed along the east side of the pad. Observed multiple damp areas on pad surface immediately adjacent to the containment. Containment nearly full with high conductivity fluid. Two samples collected. Sample results confirm pollutional substance on pad surface. Violations cited against the 3HU permit (105-21850). TLT

Report summarizes JKLM's investigation of an area that exhibited elevated soil/water conductivity, as found during DEP's 4/8/16 inspection. Based upon the results of DEP's sample collected on 4/8/16, a violation is cited against the 3HU well. The violation is immediately corrected as JKLM recovered the water as soon as being made aware. 2545913 Primary Facility 2016-12-19 Plugging(Includes Plugged/Mined Through) No Violations Noted On December 19, 2016 at approximately 10:30 am I arrived on JKLM’s Sweden Valley 102 pad after receiving Notice of Intent from the operator last week to plug and abandon (3) wells; 1HU (105-20848), 2HU (105-21868) both on the 102 pad and the 5HU (105-21858, on the 101 pad). All (3) wells being plugged are conductors only. There was a crew on site. I met with on-site representative to discuss operations on the pad and their plugging procedure. At the time of inspection the crew was digging out the first well; the conductors were cut and buried in the cellar after being buried. All the conductors are reportedly cemented in place outside the pipe and on bottom. After the cellars are dug out they’ll fill conductors with ~50’ cement on bottom (pour ready-mix) and fill with gravel from that point to surface. There are two conductors on this pad (1HU and 2HU) both 96’, and the 5HU on the 102 pad next door, set at 80’. Today they’ll dig out all three cellars in preparation for cementing operations which will likely
