2475900 Primary Facility 2016-04-07 Routine/Complete Inspection No Violations Noted Inspection to evaluate the FWI de-watering plan submitted on 3/29/2016. Plan indicates water will be discharged to the well pad surface then flow off the pad down a rock lined ditch and discharge into a small vegetated area down the slope. Erosion is a concern at several locations on the site. Along the access road and cut slope, the steep slopes are eroding. The fill slope is not stabilized and it is evident topsoil has been lost at the surface. Water flowing off the pad surface and down the fill slope has caused erosion down the fill slope east of the rock-lined ditch. The site is not permanently stabilized and has rather extensive erosion. It appears that the proposed discharge area does not meet Pa Code § 78.60(5) because the discharge area is not an undisturbed, vegetated area capable of absorbing the water. Also, the erosion caused by water flowing off the well pad raises the concern that Pa Code § 78.53 will not be met during the discharge. The Department does not recommend that the discharge

I entered the site off Tallman Hollow Road at 09:02 and began the inspection. At the time of the inspection, the Nature Boy East Yoder 1H (081-20920), Nature Boy East Unit 2H (081-20921), Nature Boy East Yoder 1V (081-21080), Nature Boy East Yoder 2V (081-21081), Nature Boy East Yoder 3V (081-21082), Nature Boy South Yoder 1H (081-21191), and Nature Boy South Yoder 2H (081-21192) were all in production.

A contractor was onsite scarifying, seeding, and stabilizing areas onsite which lacked vegetative cover or showed signs of erosion. The contractor was also re-grading the well pad so that water flows into the rock-lined ditch as designed and not over the fill slope. A sprinkler was set up on the north side of the well pad and had been watering a small section of the cut slope as indicated in the dewatering plan for the Fresh Water Impoundment. No Violations Noted.

Well has a digital gauge attached to the tubing and the production casing. The gauge on the tubing read 326 psi and the gauge on the casing read 492 psi at time of inspection. There were three anulus vents plumbed to surface. Two of the annulus vents were labeled 20 and 13 and the third annulus vent was unlabeled. There was 0% combustible gas discovered on any of the vents at time of inspection. No violations were noted at time of inspection.
