To date a total of 14 of 65 proposed total stages have been stimulated with the completion plug set on the 15th stage at a measured depth of 20,582 and the stage perforated with 6 clusters of 6 shots per foot between the depths of 20,567 and 20,398. Each stage is being stimulated with an average of 9,300 barrels of fresh water and 600,000 pounds of sand. The company is monitoring the 5.5 x 9.625 annular casing pressure however no gauges were found on the 13.375 well head section. The company men on site were advised to monitor all annular pressures while stimulating and completing operations are being conducted. |

Patterson 296 drilled a 24 inch diameter open hole to a depth of 385 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Costys ran 348 feet of 20 inch diameter casing, j-55, 94 lbs/foot with 5 feet of stick up. Casing was centralized with 2 centralizers. Baskets set at 277 feet. Coal encountered from 297-304 feet. Shoe track is 44 feet long. O-tex was onsite with 635 sacks of class H cement. Additives include 3% CaCl and 0.2% X-air. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 104 barrels of fresh water. 35 barrels of gel pumped ahead of cement with one pound per barrel celloflake. 24 barrels of cement returned to surface, and floats held with 1.5 barrels back. |

The production equipment includes 4 gas processing units and 2 glycol heaters. Upon entering the location on the left side of the well pad is the production fluid facility consisting of 6 steel construction 400 barrel fluid capacity production tanks situated inside a polymer lined steel containment dike area. Currently a service rig company identified as Mountain State pressure service is rigged up on the well drilling out the completion plugs at a measured depth of 14,350 with a well bore total depth of 23,199. A total of 64 plugs were set in the well. The well bore is static with 9.3 pound per gallon brine in the hole. No tubing will be set in the well only a tubing hanger. The company is monitoring annular pressure. |

After speaking with company man Matt Bradford, he informed me that they were currently drilling the 19th plug at 14,350 feet of 69 total isolation plugs to be drilled out. They currently had 9.3 pound per gallon fluid in the hole and well appeared to be static. The total depth of the well was 23,200 feet. I informed Matt that they needed to continue to monitor their annular pressure throughout the completion process. There were four existing producing wells on location that had steel covers over them. |

Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 30% percent excess ran., 10 feet left in the casing. Cement was displaced with 8X barrels of fresh water. 15 barrels of cement returned to surface. Water trucks were not on time. |

The casing pressure is being monitored off a electronic transducer. No annular pressure monitoring is in place. 2 Crew Men from EQT are on site replacing the emergency shut down master valve hydraulic fluid. 3 wells were recently drilled and completed and 4 pre existing wells. Production equipment consists of 5 gas processing units along with a (6) steel 400 barrel capacity steel tanks situated inside a polymer lined steel containment dike area. The well pad perimeter is protected by an earthen berm however one section was removed near the mid stream equipment and not put back. At the rear of the pad is the mid stream operation operated by EQT. I contacted ODNR Engineer Technician Anthony Carson and he will address the issue with the perimeter berm. We have addressed this issue one before and it was addressed. The Preliminary Restoration is due however recent disturbance around the well pad caused by mid stream installation shall delay the restoration until proper vegetation is established. The well pad and lease road surface is stone and in good repair. |
