2578900 Primary Facility 2017-04-03 Routine/Partial Inspection No Violations Noted On 4/3/17 at 1347 hrs I attended a preconstruction meeting for the Sweden Valley 106 well site. The following people were present: JKLM Energy representative, Jeff Long; DCNR Gas Forester, Mike Cole; and J.L. Watts representative, John Watts. Penn E&R developed the Erosion and Sediment (E&S) Control plan and J.L Watts is the construction contractor.

We reviewed the E&S drawing. Site drainage is to Nelson Run, which is designated as a High Quality stream. Approximately 8,150 feet of a pre-existing haul road will be improved to serve as the access road to the well site. The access road entrance will come off the Billy Lewis road. The site will consist of a well pad (approximately 350 feet x 500 feet) and two sediment traps; Sediment Trap 1 (ST-1) will be along the north side of the pad and Sediment Trap 2 (ST-2) will be along the west side. The pad will be soil cemented for stability and sumps will be installed in the northwest and southwest corners. The well cellars will be located within a prefabricated st

2592864 Primary Facility 2017-05-10 Routine/Partial Inspection No Violations Noted On 5/10/17 at 0942 hrs I conducted a partial inspection of the Sweden Valley 106 well site. John Watts, J.L. Watts representative, accompanied me during the inspection. Jake Long and Scott West, JKLM Energy representatives, were also present. During the inspection, I observed the access road construction and reviewed erosion and sedimentation (E&S) control documents maintained at the site.

Clearing for the access road and well pad has been completed and earthwork began on 5/4/17. The contractor was working to upgrade the pre-existing access road and was approximately at station 17+00. Site construction is proceeding as per the construction sequence in the E&S plan. High visibility fencing was in place around delineated wetlands along the access road.

Wood chips were staged along the access road to be used for construction of wood chip filter berms. The filter berms will be implemented in lieu of silt sock. Mulch will be blown along the up gradient side of the berms as per the specifications in the E&S man

2599709 Primary Facility 2017-05-24 Routine/Complete Inspection No Violations Noted On 5/24/17 at 1045 hrs I conducted an inspection of the Sweden Valley 106 well site. It was mostly sunny and 58 degrees F. John Watts, J.L. Watts representative, was on site during the inspection. I observed the access road and well pad construction and reviewed erosion and sedimentation (E&S) control documents maintained at the site.

Clearing for the access road and well pad has been completed and earthwork began on 5/4/17. The access road has been constructed to the well pad. During the inspection, the contractor was rolling the road and constructing the rock-lined channels and level spreaders. The well pad area has been cleared of trees. There were still stumps to uproot from the pad area and several windrows of stumps and other woody material to remove or chip. Two large chip piles were staged on the well pad area; the chips will be used to construct wood chip filter berms. Berms were observed in place in several areas along the access road. 2609553 Primary Facility 2017-06-29 Preoperation Inspection No Violations Noted On 6/29/17 at 1245 hrs I conducted a preoperational inspection at the Sweden Valley 106 well site. JKLM representatives, Jeff Long and John Manuel, and J.L. Watts representative, John Watts, were present. It was mostly sunny and 78 degrees F.

I observed the site and the routine Erosion and Sediment (E&S) control inspection log. Site construction is complete. E&S controls are installed and disturbed areas have been stabilized with seed and mulch or erosion control blanket; germination was noted along portions of the access road. The well pad has been soil-cemented and stabilized with rock.

Cellars have been installed for the proposed wells. According to the spud notices, the 1HU and 2HU wells will be spud on 6/30/17 and the 3HU – 6HU wells will be spud on 7/1/17.

The last routine E&S inspection was conducted on 6/24/17. No violations are cited during this inspection. I departed the location at 1334 hrs. TLT

2635121 Primary Facility 2017-09-07 Routine/Complete Inspection No Violations Noted I met with Ron Burdick of JKLM when I arrived. Tophole rig was on location running casing in the 1HU. Containment appeared to be in good condition and chemical secondary containments were neat and in good condition as well. I did note 2 small puddles (1 gallon or less) of a milky white substance in the secondary containment for the air packages, and informed Ron. It did not appear that permit numbers had been posted at the site entrance; I recommend that these be posted ASAP.
2664940 Primary Facility 2017-11-28 Routine/Complete Inspection No Violations Noted This well is top holed only at the time of this inspection. Well has a conductor that is 26" in diameter x 80' in length. A surface casing that is 18 5/8" in diameter x 622.95' in length. The 1st intermediate casing that is 13 3/8" in diameter x 2408.46' in length. There is no set date at the time of this inspection as to when the drilling of this well will continue.

Well has a conductor that is 26" diameter x80' in length. A surface casing that is 18 5/8" in diameter x 626.29' in length. Well has 1st intermediate casing that is 13 3/8" in diameter x 2607.85' in length. At the time of this inspection there was no set date to continue drilling.
2676687 Primary Facility 2017-12-28 Routine/Complete Inspection No Violations Noted A routine inspection of the JKLM Sweden Valley Pad 106 was conducted as part of a guided tour of JKLM's activity in the Sweden Valley area from 1100-1600 hours. Present for the inspection were myself and Jason Logsdon, JKLM pipeline supervisor. The weather was 8 degrees and partly sunny. The ground had approximately 3-4 inches of snow cover. 6 cellars were observed. There was no activity onsite. No violations are noted at this time.
2700278 Primary Facility 2018-02-15 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the JKLM Sweden Valley 106 well pad located in Sweden Twp., Potter Co. on 2/15/2018 @1400 hours. The weather was 48 degrees and sunny causing accumulated snow to melt and the pad surface and access road to be very wet/muddy. There are 6 wells on the pad. All wells were SPUD in late June, early July of 2017. The vertical portion of the wells was drilled with the HAD rig. The H&P 500 flex rig will be brought in to drill the lateral portion of the wells. All E&S BMPs were observed in good condition. All slopes were stabilized with vegetation where visible. The site was last inspected by company personnel on 2/12/2018 with no issues noted. No violations are noted at this time. I departed the site at 1430 hours.
2704002 Primary Facility 2018-03-01 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was top holed drilled. The coductor,surface casing and intermediate # 1 were set in the well. The conductor (25") x18 5/8" and the 18 5/8" x 13 3/8" were being vented at the time of this inspection. The annulus were tested for gas with the test results being 25" x 18 5/8" had 0 LEL and 0 Methane. The 18 5/8" x 13 3/8" had 100 LEL and 100 Methane. No other gas was detected on the wellhead assembly or the well area.

2801184 Primary Facility 2018-11-07 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted a ROUTINE inspection of the JKLM Sweden Valley 106 Well Pad located in Sweden Twp, Potter Co. on 11/7/2018 @1230 hours. The weater was 48 degrees, partly sunny, winds NNE @15mph. Joe Huston was the company representative onsite. The H&P 500 rig is currently onsite drilling 6 utica wells.

An area downslope of the flare stack had reportedly been impacted by the release of <1 barrel of drilling mud that was accidentally discharged from the stack on 10/18/2018. As of today's inspection, the impacted area appears to have been scraped, reseeded and stabilized with Erosion Control Blankets (ECB).

Violations noted during this inspection are cited as IMMEDIATELY CORRECTED. I departed the site at 1515 hours.

2818828 Primary Facility 2018-12-20 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted a routine inspection of the Sweden Valley 106 well pad located in Sweden Twp, Potter Co on 12/20/2018 @1030 hours. The weather was 34 degrees overcast. Jared Jacobini, DEP WQS was present onsite for the inspection.

The H&P flex rig had recently been removed from the site after drilling 6 Utica wells onsite. The mud tanks were being cleaned on containment in preparation for removal. The containment will remain onsite for completion of the wells, except for two opposite corners, that will be removed for installation of the AST and sand box area.

No areas of elevated conductivity were located on the pad surface. Garbage was observed down over the fill slope and in the E&S channels. This should be removed and placed in a container for proper disposal.

No violations are noted at this time. We departed the site at 1200 hours.

2823657 Primary Facility 2019-01-08 Routine/Complete Inspection No Violations Noted At the time of this inspection this wells drilling was completed on 10-16-18. At this inspection this wells pressure test for stimulation had been completed. The results of this pressure test will be compiled with the results of the tests of all six (6) wells and then forwarded all together. The tentative schedule date for the stimulation process to begin is planned for 1-17-19 but this scheduled date is not definite. No violations were noted at this inspection.
2830457 Primary Facility 2019-01-23 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was being stimulated (Hydraulic Fracturing) . This well was being stimulated at the same time as well 105-21888. This process is known as zipper fracturing and is done by most gas companies. This well has a total of 23 stages scheduled to be fractured and had completed stage 8 at the time of this inspection. No violations noted at this inspection.
2830718 Primary Facility 2019-01-22 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted an INCDT inspection of the JKLM Sweden Valley 106 Well Pad located in Sweden Twp, Potter Co. on 1/22/2019 @1130 hours. The weather was 16 degrees, overcast, winds S@9mph. Approximately 12" of snow was deposited onsite in the previous 72 hours. JKLM representative Scott West and DEP Env Trainee, Korey Starkey was present onsite. JKLM contractor Pro Frac is currently stimulating the 6 utica wells onsite. Mr. West stated at the time of this inspection they had just completed stimulating stage 13 of the 2HU well, and perforating stage 7 of the 5HU well.

Two small spills have been reported within the last week on the SV 106 well site, during the frack. Spill #1 (CTS#339324), occurred and was reported on 1/16/2019. The spill reportedly involved the release of 400 to 500 gallons of brine from the working tank. The spill was reportedly mostly on primary containment with some spray hitting the perimeter of the pad.

2841394 Primary Facility 2019-02-19 Routine/Complete Inspection No Violations Noted At the time of this inspection the Stimulation ( Hydraulic Fracturing) was complete. The first of 23 total stages was pumped on 1-16-19 and the last stage was completed on 1-28-19. No violations noted at this inspection.
2845160 Primary Facility 2019-02-26 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the JKLM Sweden Valley 106 well pad located in Sweden Twp, Potter Co. on 2/26/2019 @ 1145 hours. The weather 19 degrees, overcast, 63% humidity, winds WNW @10mph. There is approximately 2-3 inches of snow on the ground. Activity onsite consists of SPN's coil rig drilling out the plugs on the recently completed wells.

No violations are noted at this time. I departed the site at 1345 hours.
