2521338 Primary Facility 2016-09-22 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:30a.m. for a new spud inspection, Minichi Contracting Group was on site to drill the holes and run the conductor pipe. I spoke with Keith Bowman of Minichi, the rig was drilling the hole for the 3H and they were at a depth of 35' and plan on drilling to 88'. The 2H was completed and the conductor pipe was set at 88'. When the 3H is completed, they plan on drilling the 5H and setting the conductor pipe at a depth of 88'. The drill cuttings are being put into a sealed role off container and hauled to Keystone Landfill. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 10:20a.m.

2534027 Primary Facility 2016-11-02 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 8:55a.m., I met with company man Kevin Heathcock, Falcon Rig 25 was on site and waiting for the BOP test to be completed. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 1606'. Projected TD for the top hole section is 5749'. I left the site at 10:15a.m.

2564554 Primary Facility 2017-02-23 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 13:20p.m.

I met with company man John Woodburn. SDC Rig 45 was on site and drilling the lateral section of the well. The top hole section of the well was drilled and completed on 11/4/16. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 8,917'. Projected TD for the top hole section is 15,909'. I left the site at 14:05p.m.

2591764 Primary Facility 2017-05-09 Routine/Complete Inspection No Violations Noted Inspection conducted on May 4, 2017. Minor damage shown in photo 2 needs to be repaired. The road side ditch from 5+00 to 10+00 needs to be reestablished to design specifications. Photo 4 shows damage near entrance to pad. This damage needs to be repaired. Appears as if this location is a low point on the pad which is causing the damage. Please have your pad designers review to determine if controls should be added at this location, possibly a level spreader. Conklin North 1H and Conklin South 5H appear to be in production. Other three wells drilled are the North 2H, South 5H, and the North 3H. Photo 5 shows garbage that has been left on the pad. Please remove, before a storm event does.
2604199 Primary Facility 2017-06-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:40a.m.

I met with company man Harry Stevens. Halliburton was on site to stimulate the 2H well. The 2H, 3H and 5H wells were recently drilled to TD but only the 2H is being stimulated at this time. They completed stage 32 out of 43 stages and were preparing to run the plug and perf for stage 33. The water was being pumped to the site from the Zeffer production water impoundment and the Odell fresh water impoundment. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:15a.m.

2636349 Primary Facility 2017-09-15 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put into production. The well tag was on the well and correct. There are 4 other wells on the location. The 1H (115-21330) and 4H (115-20996) that have been in production. The 3H (115-22208) and 5H (115-22209) wells that were drilled to TD but not stimulated. I took a walk around the location and there were no obvious concerns noticed.

2686547 Primary Facility 2018-01-08 Incident- Response to Accident or Event No Violations Noted Gene Rickard, DEP O&GWQS and I arrived at the site at 11:29 in response to a reported spill of (300-400) gallons of production fluid to secondary containment, with (2-3) barrels of the fluid spilled escaping the containment to the pad surface. The weather conditions were cloudy skies, cold and breezy with temperatures in the 20's. US well service is the SWN subcontractor at the site in the process of conducting hydraulic fracturing (stimulation) operations.

Mike Deming, Senior Regulatory Analyst and Zachary Evans, well site supervisor/site company man, both of SWN, accompanied the Department during the inspection. Dawn Washo represented Resource Environmental of Montrose, PA. Mr. Deming and Mr. Evans provided the following details regarding the spill and site activities. "We are on stage 41 of 43 for the frac on the Conklin South 5H well. We are pressured up and perforating. The 3H is the next well to be completed. A total of (300) to (400) barrels of production fluid was spilled to containment. There were (

2688036 Primary Facility 2018-01-23 Incident- Response to Accident or Event No Violations Noted Today's inspection was conducted as an incident inspection. At approximately noon today, Mike Deming, SWN Senior Regulatory Analyst contacted the Department to inform the Department that SWN had identified stormwater discharges off secondary containment on 3 of their well pads, of which this pad is one. The discharge from containment occurred when ice formed and collapsed the containment edge. Photo 2 shows the containment edge being reestablished. The Department conducted conductivity metering and found that outside containment conductivity ranged between 150 uS and 250 uS. Inside containment readings ranged from 165 uS to 250 uS. Work over rig on site drilling out plugs. Currenty in 3H. Of note is the fact that a production fluid spill had occurred on January 5, 2018 in the photo 3 vicinity. No E&S violations were observed. No violations noted.
2722480 Primary Facility 2018-04-24 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. The battery of water tanks are still present and located on containment. All 5 wells appear to be in production. Photo 2 shows trash that needs to be removed. No violations noted.
2722788 Primary Facility 2018-04-23 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2809444 Primary Facility 2018-12-03 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Good site stabilization. No E&S issues identified. No violations noted.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2841931 Primary Facility 2019-02-14 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. As the photo's show, the site is snow covered. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
