On 2/19/09, I inspected the site and noted that uncontained drilling fluids were located on the surface of the well pad and also located off-site, downgradient of the fill slope. In addition, portions of the site were not properly stabilized in accordance with Chapter 102 regulations. The 2/19/09 inspection was inputted under # 1770903.

John Ryder accompanied me during this inspection. We verified that the previously spilled drilling fluids had been excavated as previously indicated. However, the fill slope was not properly stabilized and the silt fence at the base of the fill slope was not properly installed to minimize erosion potential. I spoke with Brad Wittrock of Chesapeake regarding the outstanding Chapter 102 violations. Brad indicated that the violations will be corrected by next week. I will reinspect to verify.

1978600 2011-06-07 No Violations Noted A complaint was filed to the Department regarding potential contamination at this site. Conducted an inspection of the site with Sean VanFleet Solid Waste Specilaist, North Central Region. We noted a small area of standing water below a cleanout in the pipeline area relatively close to th site entrance. The area was approximately 1 to 2 feet in diameter and the volume of water was approximately 2 to 3 gallons. We used an EXTECH meter to obtain a conductivity reading of the water. The meter read (OL) which means over the limit in conductivity. We performed sampling analysis of the water using SAC 046. We also noted E&S issues off the NE corner of the pad behind the production tanks. The berm was noted to contain accelerated erosion causing an erosion channel in this area. E&S issues will be referred to Gene Rickard, Water Quality Specialist, who will conduct a follow up inspection of the site.
