| At the time of inspection, I met with operator to discuss well.  There is a pump jack on the well which is hooked up to electricity.  The jack is in the down position, but operator stated that the well is pumped once a week.  He keeps the power line on the ground because of constant tree falls.  Well pumps to a tank which is uphill from the well.  Integrity appears to be in good condition, with no signs of major corrosion causing any problems.  Production hasn't been reported for well so instructions were sent via mail.  Violation from prior inspection should be removed since proof of production was given.

This discharge is an unknown quantity and the discharge has happened and or reoccurred for an unknown amount of times. The discharge is flowing downhill into a wooded area resulting in dead vegetation and erosion. This discharge also threatens to pollute the Waters of the Commonwealth. Field tests with a soil probe conductivity meter show the soil conductivity in the discharge zone and path to be 19,000 microsiemens in the production diking/containment and 12,000 microsiemens in the discharge path downhill in the wooded area.
