1843079 2009-11-09 No Violations Noted At the time of inspection, the well was properly signed. The well was drilled, but never put into production. No accelerated erosion or sedimentation issues were observed in relation to well site or access road. Area was adequately and uniformly restored with 70% vegetative cover and rock well pad. Noted, O&G inspector Paul Wyrwas determined the well to be abandoned and issued the associated violations (Inspection ID # 1838165).

2094846 2012-08-22 No Violations Noted Well tag was present. Well status was inactive. At the time of inspection, no E&S issues were present at the site.


2198759 2013-07-03 In Compliance with Policy Department contacted Operator representative Kent Keppel via phone to discuss current status of site. Department informed Mr. Keppel that site has almost reached required 70% vegetative growth and as per current regulations, Operator has until 8/16/2013 to achieve full restoration of well site.

Landowner requested that access road remains. Department recommended Operator submit documentation from landowner for verification. Operator appears to be in compliance with current policy at this time.

UPDATE: 8/28/2013 Department received above referenced documentation from landowner via Operator representative Kent Keppel.

2204235 2013-09-05 No Violations Noted Well plugged on 11/16/2012. Monument buried. Access road in good condition. No accelerated erosion or sedimentation issues observed in relation to well site or access road.

Site is permanently stabilized and adequately restored to pre-existing conditions.

As per landowner request, access road remains (See Attached).

No violations noted at this time.

2204238 2013-09-05 No Violations Noted Bond release inspection. Well plugged on 11/16/2012. Buried monument. Access road in good condition. No accelerated erosion or sedimentation issues observed in relation to well site or access road.

Site is permanently stabilized and adequately restored to pre-existing conditions. As per landowner request, access road remains (See Attached). No violations noted.

Based on today's inspection, Department is recommending bond be released for said well.
