2469298 Primary Facility 2016-03-24 Administrative/File Review No Violations Noted

On 03/24/16 I conducted an administrative inspection of the White pad in response to a spill by EQT off the White pad. When the rig was being moved on 03/15/16 at 2000 hours a mixture of produced water and drilling mud was spilled onto Bowser Road. EQT has cleaned up the spill in compliance with DEP spill policy.

2471129 Primary Facility 2016-03-14 Incident- Response to Accident or Event No Violations Noted

On 03/14/16 at approximately 1330 hours I conducted an incident response inspection in response to a spill from 03/12/16 at the EQT White pad.

On 03/12/16 at 0305 hours a brine spill occurred to the well pad surface off containment when a truck was loading and the sight glass broke. Initial estimate by the truck driver was 3-5 barrels of fluid, but EQT revised the estimate to 30-40 gallons once EQT staff were able to evaluate the spill. A vacuum truck removed standing fluid and excavation occurred at first light on 03/12/16. An area approximately 30’30’x6” was delineated with the use of a soil conductivity probe and excavated. The impacted material was placed in a lined roll off box for disposal.

2476633 Primary Facility 2016-04-08 Administrative/File Review No Violations Noted

On 04/08/16 at approximately 0800 hours I conducted an administrative review of the EQT White pad. A spill occurred at the site on 03/12/16 of approximately 30-40 gallons of produced fluid. On 03/14/16 I performed an inspection of the site and the spill area had been excavated and the material placed into a lined roll off box. The area of the spill was then filled with fresh gravel. The spill was cleaned up in compliance with DEP spill policy.

2530494 Primary Facility 2016-09-23 Routine/Complete Inspection No Violations Noted On 09/23/14 at approximately 0845 hours I conducted a routine complete inspection of the White Unit. The Act 9 sign was in place and all required documents were present at the site. There were no signs of erosion at the time of inspection. There were no violations observed at the time of inspection.

The Act 9 sign was in place and all required documents were present at the site. The production equipment appeared to be in good condition. PCSM features were installed and appeared to be functioning. The site was well vegetated and meets the requirement of uniform 70% vegetative cover. The NOT is approved.
