2553650 Primary Facility 2017-01-11 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess general site conditions. No Chesapeake representatives were present.

Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

No violations were cited during this inspection.

2596577 Primary Facility 2017-05-19 Routine/Complete Inspection No Violations Noted One well in production phase. One each sand and water separator. No leaks detected from production equipment. Site stable. No violations noted.
2791572 Primary Facility 2018-10-18 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 11:33; weather was ~50 °F, sunny, breezy.

Inspection logs were unable to be checked during the inspection due to a jammed lock on the mailbox.

There was 1 properly tagged Marcellus well staged for production onsite. There were a total of 3 well cellars filled with water; no bubbling was observed. There was 1 GPU, 1 sand separator, and 2 steel produced water tanks onsite.

The gravel portion of the pad appeared to remain at its original size. The areas around the pad were >70% vegetative cover. No E&S issues were observed.

It is recommended that storage tanks and equipment be monitored and repaired or replaced as needed.

No violations noted.
