A 2-year extension of time to restore the site was granted by the Department on March 1, 2016. This extension extends the deadline to restore the site under Title 58 Pa. C.S. § 3216 (2012 Oil and Gas Act)(relating to site restoration) until March 1, 2018. A condition of the restoration extension requires that post-construction stormwater management (PCSM) BMPs be installed by June 1, 2016, unless additional time is requested, and approved, in writing by the Department. At the time of inspection, no PCSM BMPs were in place, and no evidence such as staking or flagging were present that would indicate the imminent construction of such BMPs. Further, the Departmen't s records do not indicate that a request to extend the deadline to implement PCSM BMPs was received or approved.

2496670 Primary Facility 2016-06-20 Routine/Complete Inspection No Violations Noted Arrived @ 12:40. Weather warm and sunny. Chesapeake E&S staff were at the bottom of the fill slope. There was one producing well and a total of six slots on the pad. The slots were covered with metal plates.

Black Creek 6H, in production. Water-filled cellar, no bubbling. 2 annular vents. 13-3/8” x 9-5/8” plumbed to the dump line, -0.53 psi. 9-5/8” x 5-1/2”: shut in to gauge: -2 psi 5-1/2” casing: 18.70 psi 2-3/8” tubing: 1204 psi. Left @ 13:15

Vegetative cover on site consists of perennial vegetation with a density greater than 70%. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion and sedimentation was observed.

The post-construction stormwater management (PCSM) BMPs required by the extension of time for site restoration approved by the Department on March 1, 2016 have been installed. Erosion and sediment control BMPs were in place around the PCSM features and the areas were well vegetated.

There was 1 well onsite and a total of 6 cellars. All well cellars were covered. There was also 1 GPU, 1 sand separator, and 2 produced water tanks in secondary containment. There was a large amount of material and equipment being stored on the pad. If the equipment belongs to Chesapeake it should be removed unless land owner consent was obtained prior to storage. However, most of the material appears to be farming related (a tractor and a large number of straw bales).

The site was well vegetated with the exception of some steep rock lined slopes. No E&S issues were observed. Overall, the site appeared stable.

No violation were noted during the inspection.
