Also present was Mylen Madigan (Chesapeake). There are 4 cellars on the pad with one well in production: 2H, 113-20109. Pressures were: tubing: 1385.8 psi; 5 ½ x tubing: 276 psi; 9 5/8 x 5 ½ : -1.9 psi; 133/8 x 9 5/8 : 9.8 psi; 20 x 133/8 : N/A (no vent). For the 133/8 x 9 5/8 annulus, the ball valve was on the line to the flowback tank. To determine whether or not this could be due to a leaking check valve on the line, the ball valve was closed and the vent was first opened and then shut-in from 08:40 to 10:16. At 10:16 the pressure was 0 psi, suggesting that the check valve had been leaking.1/20/15 @14:01, OGIs Swank & Becky Renner returned following a 75-hour shut-in. Pressurees were: 1331 psi; 5 ½ x tubing: 277 psi; 9 5/8 x 5 ½ : -0.8 psi; 133/8 x 9 5/8 : 0.4 psi; 20 x 133/8 : N/A (no vent). OGI Swank collected an isobag from the tubing.
2354059 Primary Facility 2015-03-19 Routine/Complete Inspection No Violations Noted Arrived @13:40. Weather sunny and cold. With me were OGIs Ken Kennedy, Joe Gaboff, and David Schoonover. There is one well in production, on a 4-slot pad. The water in the well cellar was frozen. Pressures were: tubing: 1258 psi, on a SKADA gauge; 5 ½ x tubing: 276 psi; 9 5/8 x 5 ½ : 0 psi; 133/8 x 9 5/8 : plumbed into production tank, 0% methane. Pressure at inlet to sand separator was 1150 psi. Pressure at the production unit was 1200 psi. Left location at 14:30.

2515794 Primary Facility 2016-08-26 Routine/Complete Inspection No Violations Noted Arrived @12:48. Weather sunny and hot. This is a routine inspection. There were no personnel on site. There is one well capable of production, but shut-in, on a 4-slot pad.

MPC SUL 2H: Pressures were: tubing: 1745 psi (SKADA gauge) 5 ½ x tubing: 246 psi;

9 5/8  x 5 ½ : -3.3 psi, shut to dump line; 

133/8 x 9 5/8 : open to dump line, water filled. Left location at 13:00.

2544422 Primary Facility 2016-12-13 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. No Chesapeake representatives were present.

Permanent vegetation has been established on site and temporary erosion and sediment control BMPs have been removed.

The Department's records indicate that no current well permits exist for the site and no extension request for site restoration has been submitted or approved. In accordance with the information you provided to the Department by letter dated September 21, 2015, this site has been identified as a site for which a restoration extension request would be submitted. This information was submitted in response to the Department's June 26, 2015 letter requesting your plans to comply with Section 3216 of the 2012 Oil and Gas Act and 25 Pa. Code Section 78.65, which relate to site restoration. You are reminded that such an extension request should be submitted as soon as possible.

No voilations were cited during this inspection.

The eSubmission Post Drilling Site Restoration Report was submitted to the DEP on 8/30/2018. Based on this inspection the Post Drilling Restoration report was accepted on 9/11/2018.
