According to the Company Man on location, drilling is scheduled to commence on 12/12/09. I discussed some of the PA drilling regulations with the Company Man and provided a drillers log book.

Sand separators are installed on both wells. In cellar of well there appears to be a small leak at one of the flange bolts. DEP will notify Operator.

2688708 Primary Facility 2018-01-29 Routine/Complete Inspection No Violations Noted I inspected the Roger well pad on January 29, 2018. The pad surface was slightly soft. The access road was stable and solid. The cut slope and fill slope were adequately vegetated and stable. There did not appear to be any bubbling of the well cellars and the wells associated with this site were properly labeled. The wells associated with this site are the 115-20230 and the 115-20249.

There were no erosional issues apparent. There were no violations noted.

2711415 Primary Facility 2018-03-28 Routine/Complete Inspection No Violations Noted I inspected the Roger well site on March 28, 2018. The wells associated with this site are as follows:

115-20230 115-20249. These wells were clearly and properly labeled.

The access road was lengthy but solid and stable despite recent rains. The pad surface was a bit softer but was still stable. The cut and fill slopes were well vegetated and stable.

There were no erosional issues apparent. There were no violations.

2721806 Primary Facility 2018-04-19 Routine/Complete Inspection No Violations Noted I inspected the Roger location on April 19, 2018. I performed a RTNC inspection. The wells associated with this site are as follows: 115-20230, 115-20249. The wells were properly labeled.

The cut and fill slope as well as the detention basin were all heavily vegetated. The pad surface was a bit soft, yet stable. The access road was firm and solid. There were no erosional issues noted.

There were no violations observed.
