2443948 Primary Facility 2016-01-27 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:35 a.m., the cellar was full of ice and water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open with 0psi on it and the valve going to the tank was closed. There was 46psi on the production casing and 1166psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 12:15 p.m.

2677737 Primary Facility 2017-12-15 Incident- Response to Accident or Event No Violations Noted I arrived at the site 10:55 in response to a reported spill of an estimated (2) barrels of production fluid to the pad surface. I was accompanied by Eric Rooney, Licensed Professional Geologist, Eastern Oil and District out of the Scranton District Office. The weather conditions were mostly cloudy skies and very cold, with calm winds, and temperatures around 15 degrees Fahrenheit. There was no activity or CHK personnel at the site during the inspection. There are (6) wells on the pad in production; the Bluegrass 1H-6H. CHK initially reported the release to Eric Supey, Environmental Group Manager in the Environmental Cleanup and Brownfields Program in the PADEP Northeast Regional Office in Wilkes, Barre, PA, at 15:40 on 12/14/17. Mr. Supey reported the spill to me at 15:45 on 12/14/17. Eric Hottenstein, Environmental Manager of CHK sent an email to Matt Shope, DEP O&G WQS on 12/15/17, providing a description of the spill incident.

2690192 Primary Facility 2017-12-21 Follow-up Inspection No Violations Noted I arrived at the site at 13:06. The weather conditions were mostly sunny skies with temperatures in the low to mid 30's and light winds. Eric Rooney, DEP O&G Licensed Professional Geologist has been at the site since 12:30. As he arrived, remediation and sampling of the impacted area associated with the 12-14-17 production fluid spill was in progress. Joe Button, Senior Field Representative of CHK was at the site during the inspection conducting oversight and providing guidance to Meshoppen Stone, the CHK subcontractor performing the remediation. Mr. Button provided the following details of the spill and remediation process to the Department. "The elbow on the dump line washed out about (2) feet underground. We started excavating yesterday. We filled up (8) dump trucks with (160) tons of impacted material and placed it on containment. The production equipment has been moved. The wells are in production. Production fluid is routed to the frac tank in the corner of the pad opposite from the excavation. Electri
2701007 Primary Facility 2018-03-05 Routine/Complete Inspection No Violations Noted Frank Hoehle was on site on 2/12/2018 to conduct routine erosion and sedimentation inspection of the Bluegrass well site. Jon Sassi was on site as well, and conducted a follow up inspection connected to an incident which occurred at the site. This inspection covers only the E&S portion. Jon Sassi will be submitting a seperate inspection. The access road and pad surface were a bit soft, but not excessively so, due to recent thawing and rainfall. A broom machine was on site. The site was being excavated on the fill slope due to a recent release. Vac trucks and excavating equipment were on site and operating. The cut slope was solid and well vegetated. Approximately 5500 tons of material have been removed in addressing the issue to date and it has been trucked to Alliance landfill, as well as White Pines and Northern Tier landfills.

The erosion and sediment controls for the excavating work

2703082 Primary Facility 2018-02-12 Follow-up Inspection No Violations Noted I arrived at the site at 13:05. The weather conditions were mostly sunny skies and breezy with temperatures in the low to mid 30's. I was accompanied by Frank Hoehle, DEP O&G WQS. Nick Krut, Environmental Representative and Charlie Olmstead, Environmental Advisor and Professional Geologist both of CHK were on-site during the inspection. Leidos was on-site assisting in the guidance of the remediation and delineating the extent of the contamination of the production fluid spill discovered by CHK and reported to the Department on 12/14/17. Remediation (cleanup) of the impacted area of the pad surface and fill slope associated with the production fluid spill to the pad surface was in progress. I had contacted Eric Hottenstein, Environmental Supervisor of CHK on 2/12/18 to inquire as to when the remediation process of the fill slope impacted by the spill would commence. Mr. Hottenstein informed me that the process would commence on 2/12/18. Mr. Krut and Mr. Olmstead stated the following. "We started remediation h
2725274 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted I met with Briana Cunningham and Rob Marquez from the DEP along with representatives from Chesepeake concerning a repair underway at the Bluegrass well site. There had previously been a large release on site which permeated a considerable amount of soil. The soil had been excavated to remove the affected soil. The site was currently interim stabilized. The proposal by Chesepeake is to perform a pad reduction, reclaiming the affected corner of the pad and reducing the overall pad footprint. The earth required to fill in the remediated area is most likely to come from the opposing corner of the pad. Rob Marquez will be discussing permitting further with Brian Bailey as the ESCGP 1 is expired for this pad.

On the unworked area of the site, the cut and fill slopes were stable and well vegetated. The pad surface and access road were solid and stable.

The wells associated with this site are as follows: 115-20419, 115-20429, 115-20430, 115-20442, 115-20453, 115-20498. The wells were properly labeled.
