1991920 2011-07-28 No Violations Noted Rig is on-site. Drilling in progress. Site ID sign and E&S plan located along the access road. Pit appears to be in sound condition. There are no signs of erosion or sedimentation.
1993376 2011-08-03 No Violations Noted Drilling was ongoing at the time of the inspection. Drill pits located onsite. Pits contained drill cuttings and fluids. Pits had more than the required two feet of lined freeboard. E&S controls had been implemented as outlined in the E&S plan. Operator should continue to monitor the erosion controls to insure their continued functionality until well development activities are completed.

Met with Operator reps B. Carnahan and A. Birch to discuss well casing and cement including permanent venting of 13 3/8" x 9 5/8" annulus. Follow up meeting scheduled for 08-15-11 with Operator reps M. Jacobs, B. Wallace, and A. Birch and Department reps D. Stivanson and D. Catalano.

2000226 2011-08-24 No Violations Noted US Energy Rig#8 Speedstar building curve (7 7/8" hole) at 3061' at time of inspection. Pit lined and freeboard maintained.

Met with Operator rep W. Riggle.

2001428 2011-09-06 No Violations Noted Drilling appeared to have been completed. Two below grade pits located onsite and both have more than the required two feet of lined freeboard. There was no accelerated erosion or sedimentation observed in relation to the access road or well-site.
2013315 2011-10-18 No Violations Noted Multi Production Systems service rig swabbing oil well (post frac) at time of inspection. Pits lined and freeboard maintained.
2038105 2012-01-25 No Violations Noted Drilling had been completed, well appeared to be equipped for production. Above ground tanks located onsite and they appeared to be in sound condition. The diked area around the tanks contained no fluids. A second drill pit had been constructed onsite. The second pit had been properly lined and the contents of the first drill pit had been moved to the second pit. Second pit had more than the required two feet of lined freeboard. Phone conversation with Penneco confirmed that this was so that the first pit may be relined and reused when drilling the McCrady 5H well. All access road and site erosion controls were functioning properly and there was no observed accelerated erosion or sedimenation in relation to either the access road or well-site.

2069611 2012-05-21 No Violations Noted At the time of inspection the well appears to drilled and equipped for production. The brine tank, pump jack, and well apparatus appear to be in sound condition. No accelerated erosion observed in relation to the access road or well pad area at the time of inspection.
