Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID
2021238 2011-11-16 No Violations Noted MOVING FIRST CLASS ENERGY RIG ON TO OGM #4 PAD.
2021246 2011-11-17 No Violations Noted FIRST CLASS ENERGY RIG DRILLING ON OGM #4 PAD, DEPTH OF 127¿, WAITING ON CEMENT.
2022449 2011-11-29 No Violations Noted rilling had been completed well was not equipped for production. Drill pit located onsite and it had more than the required two feet of lined freeboard. Areas of disturbance that will not be addressed until final site restoration required temporary stablization as per 102.22 (i.e. seed & mulch). This action along with the installed erosion control BMP's will minimize accelerated erosion & sedimentation. At the time of the inspection there was no observed accelerated erosion or sedimentation in relation to the site. The access road shared with OGM 3 requires maintanance as large chunks of mud are being transported off-site onto the paved road and subsequently onto Frye Hollow Road. WQS Higgins contacted Oil & Gas Management via phone at the office and was informed that they are aware of the situation and that a plow/broom truck is en route and that as soon as the weather permits the areas of concern will be seeded & mulched
