1997890 2011-08-22 No Violations Noted United drilling Rig 58, Comp. Man - Jim Quesenbury, Drillers log - checked, drilling at 660'. Cellar - checked.

Had a problem with water at 380', semmed to think it was rain water not ground water. They grouted the mousehole and water stopped. 0 - 40' chunk rock/dirt 40 - 100' hard green rock 100 - 300' red shale 300 - 600' sand, shale(gray) soupy hole from 90 - 100' 20" csg @ 100' W/ 10 yds. I went to site for an inspection and found they had just reported a spill. Rig was shut down while they cleaned up spill,. Approximately 3 bbls of water based drilling mud(fresh water with gel) flowed out of cellar onto containment. They had placed a pump in cellar and put discharge line in cuttings tank, but no one manned the pump and it did not operate correctly. Spill was cleaned up immediately with no fluid off containment. Cause of fluid, mud coming up outside of conductor which is not cemented.

2004607 2011-09-20 No Violations Noted UDI Rig 58, checked cellar - good

Well is done, moving out. Watch freeboard on tanks. Also, as equipment is being moved, containment tears. As crews pressure wash equip., be sure to vacuum up dirty water so it doesn't get off containment.

2008940 2011-10-06 No Violations Noted Cellar has been filled in, 2 vents go to 550 gal poly tank.

2H is written on well head but the sign on it says 4H. 1 dumpster still on site. A few small pieces of plastic and felt are laying around. Generally, the site looks good.

2026238 2011-12-09 No Violations Noted Well has been fraced. Plastic containment is still down. Frac tanks on site. Cellar is full of gravel, but there is water on top - no bubbles. There are 2 annular vents w/ 0 psi - go to poly tank. Gas meter detected 0 gas in tank. 1600+ psi and 1750 psi on production guages. This is the only well on site, unless there are any under the plastic.
2029271 2011-12-15 No Violations Noted On site to check annuli for gas, because of water complaint from nearby landowner. Eric Rooney was on site to take isotopic samples. Rich Adams, Ralph Larsen, and Jon Hafer were on site representing Chief O&G. Checked 2 annular vents and got readings 0 psi and 0 comb. gas on both.
2029273 2011-12-09 No Violations Noted Well has been fraced. Plastic containment is still down. Frac tanks on site. Cellar is full of gravel, but there is water on top - no bubbles. There are 2 open aqnnular vents w/ 0 psi that go to a poly tank. Gas meter detected 0 gas in tank. 1600+ and 1750psi on prod. This is the only well on site, unless there are any under the plastic.
2104994 2012-10-04 No Violations Noted The operator reported a release of approximately 2 barrels (84 gallons) of flowback fluid to the cellar when a small leak was detected. Once discovered the fluid was immediately pumped out. The release occurred on 10/3/2012.

I arrived on site at the conclusion of the fracking operations. A new frachead had just been installed. I met with the site foreman before conducting the inspection. I examined the cellar where the release had occurred. A vacuum truck was on station to pump out any residual fluid that may arise with the rainwater entering the cellar.

There was no evidence that any material had left containment. The cellar is composed of impermeable material and is surrounded by containment.
