| Incedent investigation:  Incedent occured 3/11 in evening.  Two men entered frac tank to clean(did not follow confined entry protocal). Fracing was going on on the pad.  After perforating stage 4, fluid( fresh water) and pressure on the lubricator was flowed back into the same frac tank.

Frac Inspection:  Just finished perforating and are on way out of hole for stage 8/30.  Halliburton is frac company.  Comp Man is Morgan Knapp.  Walked around site.  They are using a Stinger Wellhead Isolation tool to protect wellhead(also called tree saver).  Also using Sierra Dust Control system .  Discussed some minor containment problems.
