2633527 Primary Facility 2017-08-22 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 12:16. I met with Nick Krut, Environmental Representative of CHK. The intent of the inspection was to respond to the spill incident that occurred at this site on 8/17/17. Mr. Krut stated the following in reference to the spill. "A (130) barrel capacity vacuum truck was in the process of delivering freshwater to this pad to be offloaded into the Modular Aboveground Storage Structure containing water from well development impoundments and water withdrawal sites. The water in this structure is transferred via a pump to a water transfer line on this pad and eventually transferred to the CHK DPH pad to be used during hydraulic fracturing. The truck was parked near the water storage structure when the spill occurred. The hydraulic line failed on the truck near the connection point between the hose and fluid reservoir releasing approximately (20) gallons of hydraulic fluid onto the pad surface. The company responsible for the spill was Fenton Welding Transport of Wyalusing, PA. They mobiliz
