0027245666 2016-08-15 Production Wells No Violations I received a call Charlie Youngen, foreman for Enervest informing me that a leak in the flowline was discovered and contamination had entered a stream. On site were DOGRM Fred Romeo and Josh Scheiring, as well as representatives from the Muskingum Watershed Conservatory, the Carroll County EMA, and the Ohio EPA. The spill occurred when a flow line from the wellhead to the tank battery ruptured west of State Route 542, and flowed crude oil into an unnamed dry ditch, travelled into a culvert and crossed to east of State Route 542, and then into a perennial tributary of Atwood Lake. Total volume of the spill is estimated to be four bbl. The flow in the perennial stream was blocked by a log which acted like a siphon dam and blocking the oil from traveling into Atwood Lake. Booms were deployed to prevent further spread. A crew was in the stream with diapers soaking up free product. The leak has been repaired. A vacuum truck was on site removing free oil from a collection point at the road culvert.

1576678285 2015-09-01 Production Wells No Violations I conducted a status check of this well. The well is equipped with an electrically-driven pumpjack. The well was producing at the time of inspection. The tank battery shares production equipment with PN# (20411, 20417) and consists of one 210 barrel production tank, one vertical separator, one underground cement brine vault, one drip bottle, and one digital meter run. The tank battery is surrounded by a dike. The tank valves are sealed with a plug. There is a sub-meter located outside of the tank battery. Identification is posted at the well and tank battery. The access road and site are stable. No violations were observed.
2060724537 2016-08-16 Production Wells No Violations I was on site to follow up on remediation for the flowline leak which was discovered on 08/15/2016. On site were Enervest personnel, DOGRM Josh Scheiring, and representatives from the US Army Corps of Engineers. Enervest personnel were continuing manual cleanup in the stream with diapers. Contractors for Enervest were excavating all contaminated soil out from the impact site located west of State Route 542. A minor amount of residual product continued to accumulate at placed booms.
