1716182951 2015-12-21 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The wellhead had a pumpjack on it and the valves were open. The tank battery consisted of one, 140 BBL production tank and a mechanical separator. The chart in the chart box showed production. The tank battery is diked. The well is properly marked with a clear, legibley marked well identification sign. The wellsite and tank battery locations were stable and vegetated. The lease access is stable and driveable. Preliminary restoration passed.

2132065495 2016-08-10 Production Wells No Violations I conducted a status check of this well. The well is equipped with an electrically-driven pumpjack. The production valves were in the open position. The tank battery consists of one 210 bbl production tank, one vertical separator, one drip bottle, and one meter run with a current 31-day chart. The production tanks load line valves was secured with a plug. The tank battery was surrounded by a dike. Identification was posted at the well and tank battery. The lease road was stable.
