1321824861 2017-09-25 Production Wells No Violations No changes have been made to the well or pad since my last inspection. The well is in production and no violations were observed at the time of inspection.
1418458055 2016-06-20 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or brine water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

one sand separator one processing unit one heat treater one 50 bbl tank labelled as containing sand six 400 bbl tanks, three labelled as containing brine, and six labelled as containing condensate

Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a chain link fence. Drainage and erosion controls at the pad are in place and functioning. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1879666067 2017-05-22 Production Wells No Violations I conducted a status check of this pad. The well is in production and equipped with an open flow wellhead assembly. Production equipment on site consists of:

12- 210 bbl production tanks; six labelled as containing condensate, and six labelled as containing brine, 1 gas processing unit 1 vertical separator 1 horizontal separator 1- 50 bbl steel tank labelled as containing sand. The tank battery is surrounded a steel dike and E stops were present. The pad is surrounded by a six foot high barbed -wire fence. The man doors were not locked but Columbian Gas personnel were on site. No violations were found during my inspection.
