0049108613 2017-11-08 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0615. The access and locations were stable. A review of RBDMS indicated production was current. Correct identification was posted at the well head. The 8.625 casing was below grade. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. Staining consistent with a legacy operation was present. A domestic supply connection was present. The tank battery was shared with #2-0616 and consisted of a vertical separator that remained in good repair and contained a current production chart. Tanks inside the dike consisted of two (2) 100 bbl production tanks that were manifolded together. The West tank remained in good repair with closed and plugged valves. The East tank body remained in good repair and contained correct identification. I observed residual production fluid staining at the base of the tank. The fluid was not fresh or live. Valves were closed and plugged. A worn earthen dike was present around the equipment. I spoke with Enervest production foreman Joe Teppert concerning the worn dike and status of the tank. Mr. Teppert stated he would schedule improvements to the dike and would have the tank evaluated. I will schedule a follow-up inspection in 30 days to document the status of repairs.

0335530653 2003-06-26 Production Wells No Violations On 06/24/2003 received a call from Mrs. Pat Tuel that the separator across the road had failed again the night before. No problem now. On 06/25/ 2003 called Mr. John Ferco of Power Gas Marketing of the problem. Mr. Ferco is to check it out and call Mrs. Tuel. On 06/26/2003 met with Mr. Dale Tuel to discuss the problem with the separator across the road. Showed Mr. Tuel where the problem has been and how to correct the problem until the company can get there to make more permanent repairs. There was no leakage during the inspection. Recommend case be resolved.
1052749813 2003-05-10 Production Wells Violations Noted Received a call from Mrs. Pat Tuel that the bad odor and hissing was back again. Arrived on site and found that the control valve on the side of the separator was causing the system to partially bypass the sale line and vent into the oil storage tank. Called Mr John Ferco of Power Gas Marketing. Obtained permission to pull a battery out of each logic box for the number one and two wells. Met with Mr. Wartluft on site and showed him the problems and what could be done to correct the problem. Mr. Ferco is to replace the bad valve Monday.

1799829555 2014-08-14 Production Wells No Violations I was on site to conduct a status check. Identification was posted on the tank battery. The tank battery is a shared battery with #20616 (Wartluft #2). The tank battery consists of one 100 bbl production tank, one 100 bbl brine tank, and one vertical separator. The tank valves are plugged. A meter with a current chart showing production was observed. A dike is present. The wellhead was equipped with a plunger lift and domestic supply line. The access road is in good repair.

2023659227 2003-04-30 Production Wells Violations Noted Received a call from Mrs. Pat Tuel of 22840 Buck Road, Alliance, Oh. 44601. Telephone 330-821-5918. The complaint was of odor and hissing noise all weekend long. Arived on site and met with mrs. Tuel. Found that there had been a fluid leak from a nipple between the storage tanks had been replaced. Oil had run down the side of one tank and puddled on the ground around the west side of the tank. Sorbent pads had been put down but not removed. Found no violations a the wellhead and no unusual odors or hissing noise. Gave Mrs. Tuel my business card and asked her to call if she had the problem again. Contacted the company Power Gas Marketing in Garrettsville and informed them of the complaint. A work crew is nearby and will be sent over to remove the contamination and walk the flow line to check for any leaks. Case pends.
