0480965202 | 2018-10-30 | Field Inspected, Well Not Found | No Violations | The purpose of my inspection was to document the status of well #2-0781. At the time of my inspection I did not locate a well head, observe any surface expression, or see evidence of any production equipment at the identified location. I reviewed the completion report in RBDMS. The completion report indicated that well #2-0781 was plugged and abandoned on 09/27/1980. Columbus RBDMS reported I.P. P&A in the master well file comments. The well status was still listed as producing. Enervest, the current owner, has this well permit number mis-identified in the field on producing well 029-2-0853. Prior to the inspection, I spoke with Enervest field superintendent Joe Teppert regarding the situation. I will communicate my findings to ODNR-DOGRM Northeast Regional Manager Thomas Hill. |
1874704570 | 2017-11-02 | Production Wells | No Violations | The purpose of my inspection was to conduct a status check on well #2-0781. The owner was current with reported production. Access to the well was stable and drivable. I met Enervest Superintendent Joe Teppert on location. The well location was stable. The 4.5 production casing was equipped with an electric pump jack. Production valves were open. No leaks were observed. Staining consistent with a legacy operation was present. A domestic supply connection was present. Production equipment consisted of one (1) horizontal separator, one (1) 100 bbl production tank and one (1) concrete brine vault. The separator remained in good repair. The separator did not contain a production chart. An earthen dike was constructed around the production tank and vault. Mr. Teppert was on location to evaluate the West side of the dike that was being eroded away due to surface runoff. Mr. Teppert stated he would schedule a contractor to make repairs to the dike and prevent further erosion. The production tank remained in good repair and contained correct identification. Valves were closed, plugged and grounded. The brine vault remained in good repair above grade and was holding fluid that was near free board. I will schedule a follow up inspection to document the repairs to the earthen dike. |
1992708562 | 2018-04-04 | Production Wells | No Violations | The purpose of my inspection was to document the condition of the earthen dike around the tank battery of well #2-0781. At the time of my inspection the earthen dike had been repaired and remained stable and capable of containing a release. No further action is necessary. |
