1491222720 2017-03-21 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0944. A review of RBDMS indicates the Owner is current with reporting production. Lease access to the well and tank battery was stable and drivable. The well was located on a fill slope within an agricultural field. The location was stable. A gravel apron was in place around the well head. Correct and legible identification was present on the electric box. The 8.625 well head was below grade. A riser plumbed to the annular side port was plumbed above grade and open to atmosphere. The 4.5 production casing was equipped with an electric pump jack. The stuffing box rubber was leaking crude oil to the gravel apron around the well and staining the gravel. No fluid was pooling or migrating off of location. A casing pressure gauge indicated 35 psi. A tubing pressure gauge indicated 20 psi. I also observed production fluid to be leaking from connection on the production side flow line at the well head. The fluid was staining the gravel apron under the connection. No fluid was pooling or migrating off of location. The tank battery consisted of one (1) horizontal separator, one (1) 210 bbl production tank and (1) buried concrete brine vault. The horizontal separator was positioned outside of the earthen dike. The separator was in good repair. A current 31 day production chart was in the meter house. An earthen dike that was stable and free of production fluid was present around the production tank and buried brine vault. Correct and legible identification was present on the tank body. No holes or leaks were observed on the tank body. The load line valve was closed and plugged. The drain line valve was closed and plumbed out to the brine vault. The hatch lid was closed. The brine vault was in good repair above grade. No evidence of overflow existed. Minimal fluid was contained in the vault. While on location, I called and spoke with Enervest lease operator Steve Pegg concerning t
1491850646 2017-04-03 Production Wells No Violations The purpose of my inspection was to follow-up on repairs to leaks detected during my previous inspection. The stuffing box head was cleaned of free production fluid. The packing nuts on the end of the production flow line were cleaned and did not show evidence of leaking. While on location, I called and spoke with lease operator Don Pahovey who stated that the stuffing box was adjusted and the packing nuts were tightened. No further action is necessary.
