0152289770 2006-01-13 Production Wells Violations Noted The tank battery consists of 1-210 bbl storage tank, 1-buried concrete salt water tank and a dike. The wellhead bradenhead is leaking gas.
0189013960 2017-10-20 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1155. A review of RBDMS indicated the owner was current with reporting production. The access and location were stable. Correct and legible identification was present. The 8.625 casing head was below grade. The 4.5 production casing was equipped with an electric pump jack. I observed leaks and staining consistent with a legacy operations. The staining was collecting on the ground beneath the well head flow line. Production equipment on location consisted of one (1) horizontal separator, one (1) 210 bbl production tank, one (1) concrete brine vault and one (1) meter set. The horizontal separator was positioned outside of the earthen dike. The separator remained in good repair. No leaks were observed. A stable earthen dike that was free of fluid was constructed around the production tank and brine vault. The production tank remained in good repair. Valves were closed and plugged. The brine vault remained in good repair above grade. No evidence of overflow was present. The tank was holding fluid with freeboard. The meter set contained a current 16 day production chart. While on location I spoke with lease operator Don Pahovey who stated he would inspect and clean the well head staining within 7 days. I will schedule to follow up with Mr. Pahovey in one week.

1343452624 2014-10-15 Production Wells No Violations I was on site to conduct a status check. Identification was posted on the tank battery and wellhead. The tank battery consists of one 210 bbl production tank, a brine vault, and one horizontal separator. The tank valves are plugged. There is a meter box with a current chart showing production at the access. The dike is present and in good repair. The wellhead is equipped with an electric pumpjack. The access road is gated and locked. The access road and site are stable.
1794484870 2017-10-23 Production Wells No Violations The purpose of my inspection was to follow-up on repairs to well #2-1155. I received a phone call from Enervest lease operator Don Pahovey concerning the staining on the well head flow line. Mr. Pahovey reported that the connections were tight and the staining was old. He continued that while on location he cleaned the connections and collected the stained material from the ground. During my follow-up inspection, I witnessed the connections to have been cleaned and stained ground to have been removed. No further action was necessary.
