0112999902 2017-08-17 Production Wells No Violations The purpose of my inspection was to document repairs to the leaking well-head and production tank access door identified in compliance notice # 1675462409. On August 7, 2017 I received a phone call from Joshua Schrock with Enervest Operating stating that the well-head leak had been investigated and material to repack the well-head were on order. Additionally, the leaking access door gasket would be addressed by a roustabout crew at that time. On August 11, 2017 I received a follow-up phone call stating repairs at been completed. At the time of my inspection, I did not observe any well head leaks. The access door gasket had been replaced. No additional violations were observed during this inspection. No further action is necessary. I will close compliance notice 1675462409.

1675462409 2017-07-05 Production Wells Violations Noted The purpose of my inspection was to conduct a status check on well #2-1356. RBDMS indicated the operator was current with reported production. The road, well and tank battery sites were stable. Correct and legible identification was present at the well head. The 4.5 production casing was equipped with a plunger lift assembly. The 8.625 well head was leaking gas and production fluid onto the ground. The contamination was contained to the area immediate to the well head. No fluid was migrating away from the location. A valve on the tubing string was also leaking oil. The tank battery consisted of one (1) horizontal separator and one (1) 210 bbl production tank. An earthen dike that was stable and free of fluid was present around the equipment. The separator remained in good repair and contained a current 16 day chart. Correct and legible identification was present on the tank body. A patch on the heater tube was failing and leaking production fluid onto the ground. No fluid was pooling or migrating. Valves were closed and plugged. I called and spoke with Enervest field manager Joe Teppert on 7/6/2017 concerning the well head and heater tube leaks. Mr. Teppert stated he would have the lease operator assess the leaks and make necessary repairs as soon as possible. Operation of well #2-1356 was causing pollution and contamination. Enervest Operating LLC shall make necessary repairs of the well and production equipment such that operations are conducted in a manner which will not contaminate or pollute the surface of the land, or water on the surface or in the subsurface per OAC 1501:9-0-07.
