0160007802 2018-08-09 Production Wells No Violations The purpose of my inspection was follow-up on compliance notice # 1302165157 and complaint #5464. While on location I called and spoke with Enervest field superintendent Tim McConaha. Mr. McConaha stated that he planned to be on location to plug the well before the end of 2018. The existing plug and abandon permit is valid until 6/22/2019. I also called and spoke with landowner and complainant Gary Irwin. I updated Mr. Irwin on the status of scheduling the work. Correct identification was present at the well head. The 4.5 production casing was equipped with a 4.5X2 swage. Valves remained closed. No leaks were observed. I will schedule to be present during plug and abandonment operations.
0452328529 2018-11-01 Plug / Plug Back No Violations The purpose of my inspection was to follow-up on compliance notice # 1302165157. At the time of my inspection I witnessed plug and abandonment activities on well #2-1464. Appalachian Well Services was on location to shoot off the 4.5 production casing. The 4.5 casing was shot at 3350 below surface. One joint of casing was removed. Chipco established circulation and pumped a 60 sack Big Lime plug. After setting the Big Lime plug, Chipco pulled 78 joints of 4.5 production casing up to 784. While pulling casing from the well bore I met with property owner Gary Irwin and Enervest representative Tim McConaha. Mr. Irwin had filed complaint #5464. Mr. Irwin and I discussed the scope of work and agreed the complaint could be closed. I explained to Mr. Irwin the requirements for restoration and he and Mr. McConaha discussed the details of what would be done and what would be waived. Once the 4.5 casing was at the required depth, circulation was established and a 60 sack plug was set across the 8.625 surface casing seat. After setting the surface casing seat plug, the 4.5 production casing was pulled up to 150 below surface. Circulation was established and a total of 60 sacks was pumped with cement circulated to surface. The remaining 4.5 production casing was removed from the well and topped off with cement to surface. The cement held at surface. I will schedule a follow-up inspection to document the status of the plug.
0919471580 2018-10-31 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #2-1464. Inspector John Fleming was also on location. Enervest field superintendent Tim McConaha was on location as the company representative. Chipco was the plugging and cement contractor. The top of the Clinton isolation plug was tagged at 4719 below surface. Chipco rigged up the casing ripper and when attempting to rip casing the tool became separated. Operations were delayed while a new rip tool was delivered. After making up the new tool, Chipco unsuccessfully attempted to rip the 4.5 casing. Mr. McConaha then scheduled Appalachian to bond log and shoot the casing off the following day. I will schedule to be present to witness the remaining plug activities.
1302165157 2016-11-01 Production Wells Violations Noted The purpose of my inspection was to investigate a complaint (Complaint #5474) called in by surface owner Mr. Gary Irwin. Mr. Irwin stated that the well is not in production and approximately three years ago the well owner (Enervest) removed the production equipment and tank battery. Mr. Irwin also stated the well is incapable of supplying gas for domestic purposes. A review of RBDMS indicates the Owner has not reported production since 2012. Access to the well head is through a driveway utilized for commercial business and remains in good repair. The well location is within an area utilized by the surface owner for equipment storage. The location lacks stabilizing vegetation. The location lacks relief preventing erosion and sedimentation. Correct and legible identification is posted at the well head. The 8.625 well head is below grade. The 4.5 production casing contains a 4.5 x 2 swage and ball valve that is plumbed to a drip. The drip is plumbed into a domestic supply connection. A gauge on the supply line indicates 5.5 psi. While on location, I called and spoke with Enervest Superintendent Tim McConaha regarding plans for the well. Mr. McConaha stated he would check on future plans and communicate back on his findings. Mr. McConaha called back the same day and stated that the well was watered off and required plugging. The well is currently incapable of production. Enervest shall submit a written plugging schedule to

Inspector Kenneth Frye at the Ohio Department of Natural Resources, Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685, within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. Enervest will be notified if the plugging schedule is approved or denied.

1497404227 2018-11-02 Plug / Plug Back No Violations The purpose of my inspection was to document the status of plug and abandonment activities on well #2-1464. At the time of my inspection the depressed area around the well head was covered in meteoric water. No gas was observed leaking from the well. I will schedule a follow-up inspection to document the status of final restoration.
1541175509 2018-11-02 Plug / Plug Back No Violations The purpose of my inspection was to document the plugged status of well #2-1464. At the time of my inspection, the location around the well head was covered with runoff from recent precipitation. I was unable to determine if the plug had stayed at surface or not. I will discuss the situation with Enervest field superintendent Tim McConaha. The location remained stable.

Chipco rigged up on the well and installed 1.5 tubing into the well to place the producing zone isolation plug. Circulation was established and 30 sacks of Class A cement were pumped. After the plug was displaced to depth, operations ended until the following day to confirm the plug placement and integrity.
