0081591351 2017-10-26 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1516. A review of RBDMS indicated the owner was current with reported production. The lease access and locations were stable. Correct identification was posted. The 8.625 well head was below grade. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the open position. No leaks were observed. Staining consistent with a legacy operation was present. The tank battery was shared with #2-1498, 2-1499, 2-1515, and 2-1517. Equipment on location consisted of two (2) horizontal separators , two (2) 210 bbl production tanks and one (1) 800 gallon poly brine tank. The separators were not identified as to which well they operated. Both separators remained in good repair and contained current production charts. A worn earthen dike that was free of fluid was present around the production tanks and brine tank. The production tanks remained in good repair and contained correct identification. Valves were closed and plugged. The brine tank remained in good repair with no cracks or leaks. The inlet was placed in the top of the tank. No violations were observed during this inspection.
0193257923 2005-12-08 Production Wells Violations Noted The tank battery is shared with permits: 1499, 1517, 1515 and 1498 and consists of 2-210 bbl storage tanks, 1-1,100 gallon plastic salt water tank and a dike that is breeched on the west end. The stuffing box is leaking and need to be repaired and/or repalced. Identification at the tank battery is faded and incomplete.
1396968989 2016-03-03 Production Wells No Violations I received a phone call from Joe Tippert stating that they had a polish rod liner leak and that there was a small amount of oil/brine sprayed around the front of the pumping unit and collected into a small ditch. John Rose was on location with a backhoe removing the contaminated soil. J Hawk Water Service was on location to pick up what oil/brine that drained into to the ditch. Terry Whitehair was contacted.

2112129304 2016-03-04 Production Wells No Violations I performed a follow up inspection on the above location. All contaminated soil has been removed and disposed at an approved land fill.
