0701432071 2018-02-09 Non Well No Violations *This report was entered under permit 5276 because it is the nearest well to the investigation site* On the day of my inspection I was traveling south on SR 83 when I noticed what appeared to be crude oil floating in Mill Creek, just south of the confluence with Spoon Creek. I stopped to investigate and found that there were several blocks of a brown, slightly foamy substance floating on the water. They ranged in size from approximately 3 feet in diameter to 3 inches. The blocks were caught in the ice and did not appear to moving downstream. There was heavy snow cover on the ground and the banks of the creek were sheer drop-offs. I walked downstream and found no signs of the substance. I walked upstream and found that it was coming from Spoon Creek. There were small blocks of brown foam floating down Spoon into Mill Creek and collecting in the ice and woody debris just south of the bridge. I called Coshocton County EMA Director Rob McMasters and he came out to meet me at the site. We both walked the area and agreed that we did not see any of the sheen on the water that is commonly associated with crude oil spills. There were no visible flow lines around the area. The nearest well was over 700 feet away and there were no signs of oil leaking from the production tanks. Rob suggested that the substance was biological foam that frequently appears on the creeks and rivers in this area. He took photographs and sent them to Troy Cole, Deputy Director of the Coshocton County Homeland Security & Emergency Management Agency. The consensus between Rob and myself was that the substance was not crude oil. I drove east on CR 193 to the point where it crosses Spoon Creek and I did not find any sheen on the water or signs of oil in the snow around the local wells.

1067206067 2016-01-25 Production Wells No Violations On the day of my inspection I found that the wellhead was equipped for open flow/swabbing. The casing valve was open and the flow line was connected. The tank battery consisted of one 100 barrel tank, a vertical drip bottle and an earthen dike. Identification was posted and I found no leaks or spills.
