0092238248 2008-09-23 Preliminary Restoration No Violations Lease road is stable and constructed out of durable material. Well is a rabbit well. 100BBL tank and a poly tank in battery. Mechanical separator. Dike is in good repair. Dike has a 4 inch PVC pipe with an elbow and extension on it. Elbow was down, it would allow any fluid in the dike to drain. Moved the elbow to a 90 angle so inlet is in the air. Vegetation is established on site.

Pits are in good repair. Drill depth 378. Asked Jim to take samples of the Berea, at least to tell where the bottom of it is. Operator is going to set 12 joints of 8 and 5/8, 480.
1199291234 2007-12-28 Drill / Deepen / Reopen No Violations On site for surface casing cement. Hole #1 44 in depth and 11 inches in diameter. Conductor set at 44 feet, driven in. Conductor pipe was 11.75 inch new 32 lb pipe. Surface hole is 524 feet in depth, 11 inches in diameter. Hole was drilled to 270 feet on air. Operator switched over to fluid drilling when hole began making water. Operator switched back to air when hole got tight in the Berea Sandstone. Berea depth per the driller 400 feet to 420 feet in depth. Cement company (Universal,) ran 65 barrels of fresh water, 300 LB of gel and a 5 barrel spacer, circulation was not obtained. Operator ran 150 sacks of cement no circulation of cement. Cement was ran without inspector approval. I spoke to Glenn from Universal and Jim from Union drilling after the job. I inquired what there logic was to run cement without gaining circulation. Glenn stated, that the tool pusher Jim gave approval to run cement without ganging circulation. Glenn stated, that by increasing the pump rate when pumping cement, he was hoping to bridge the section of hole that was taking fluid. Thus allowing for return of cement to surface. Jim was requested to tag the cement before drilling. 12/28/2007 6:32pm I called John, the Consultant Geologist for Power Gas Marketing and Trans and informed him of what had occurred. John was informed that cement should not be pumped without ganging circulation or further attempts being made to gain circulation. John and I agreed that the cement either needs to be tagged of a bond log ran to find the depth of cement. 12/28/2007 6:34pm I called Joseph Schwab from North Coast Energy Inc. And informed him of what had happened. 12/29/2007 Joseph Schwab called me and informed me that the cement was tagged at 90. Operator will grout from the top when cementing long string.

Valves on the separator were open and the chart showed production. The tank battery was diked and had legible signage. The lease access was stable. No violations observed.
