0059932260 2015-09-25 Plug / Plug Back No Violations On the day of my inspection I witnessed that top cement plug had stayed at stayed at the surface with no migration of gas. Chipco had removed the 8.625 inch casing wellhead placing top of casing at 5 feet below grade. Chipco installed a metal plate with the 4 digit permit number identifying the well. Chipco then rigged down and began site reclamation.
0374552060 2015-09-15 Plug / Plug Back No Violations On the day of my inspection I witnessed Chipco LLC was not able to trip in the 4.5 inch casing past 2800 feet with their tools due to casing being collapsed. Appalachian Well Service was called out and tripped inside the 4.5 inch casing with a 4.5 inch casing cutter and found a free point at 2792 and cut the casing. Chipco freed up casing gained circulation and pumped a 90 sack 300 foot Class A plug from 2792 feet to 2392 feet. Cement was displaced with 39bbls of water. Chipco tripped 4.5 inch casing to 1441 feet gained circulation and pumped a 60 sack 200 foot Class A bottom of the surface plug from 1441 feet to 1241 feet. Cement was displaced with 10.1bbls of fresh water. Chipco tripped 4.5 inch casing to 200 feet gained circulation and pumped a 70 sack 200 foot Class A surface plug from 200 feet to surface.
0475692346 2015-09-24 Plug / Plug Back No Violations On the day of my inspection I witnessed Chipco LLC had successfully drilled through the surface plug and tagged the bottom of the surface plug at 1230 feet. Chipco tripped out and laid down drill bit and 6 inch drill collars. Chipco tripped back in with 2.875 inch tubing to 1205 feet. Chipco gained circulation and pumped a 60 sack 200 foot Class A plug from 1205 feet to 1005 feet. Cement was displaced with 5.8bbls of fresh water. Chipco tripped out tubing and topped off casing and verified there was no gas migration. Chipco tripped 2.875 inch tubing back in to 300 feet and pumped an 86 sack 300 foot Class A cement surface plug from 300 feet to surface. Chipco tripped 2.875 inch tubing out and cement was at 7 feet. Chipco topped off cement and cement stayed at the surface with no gas migration.
0675774043 2015-09-22 Plug / Plug Back No Violations On the day of my inspection I witnessed Chipco LLC was drilling out the top cement plug with a 7.875 inch button bit , 2- 6 inch drill collars and 2.875 tubing at 145 feet. The pit was constructed and lined with plastic to the flow ditch. Chipco will drill through the surface plug and tag the bottom of the surface at that point re-plugging will commence.
0705688121 2015-09-14 Plug / Plug Back No Violations On the day of my inspection I witnessed Chipco LLC gained circulation and set a 400 foot Class A Clinton plug from 5100 feet to 4700 feet. Cement was displaced with 18.2bbls of water through 2.375 inch tubing. Chipco was unable to trip tubing from well and Appalachian Well Service tripped inside 2.375 inch tubing and could not get down past 3210 feet. After multiple attempts to get through the 2.375 tubing it was shot off at 3210 feet by Appalachian Well Service and Chipco tripped out the tubing and will rip 4.5 inch casing tomorrow.

1442406367 2015-09-16 Plug / Plug Back No Violations On the day of my inspection I witnessed that the surface plug had dropped and a 3 inch diameter hole was present with gas migrating through it. During inspection Chip Smith of Chipco LLC called and stated that the plan is to drill out the cement and set new plugs accordingly. Chipco will notify when they are ready to drill out. Readings taken with ODNR Honeywell PhD6 meter revealed a LEL of 3 at the time of inspection.
1657235258 2015-06-26 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with 8.625 inch casing, 4.5 inch casing, 2.375 inch tubing, a plunger lift system and domestic gas service with all valves closed. The production equipment included a 50bbl tank, a vertical separator and a 2 inch sales meter with a .625 inch orifice and a chart recorder. Well identification was present and well is scheduled to be plugged with Chipco moving in next week.
